Harmonize license file formatting

To quickly check if all projects 1) have a license file and b) it is the
MIT license it helps if all files follow the same format and
conventions.

The following changes were made (if applicable):
1. Rename various variations to LICENSE
2. Use "MIT License" as the first line of the file
3. Copyright line follow the header
4. Put the line breaks in the same place.
5. Break lines using LF not CRLF.

Note: This commit does not meaningfully change the license.
