docs: the incident-response plan adds two held requirement lines and the ISO/IEC 27090 line, and previews bind their own status namespace - #104
Two held requirement lines and one forward line in the incident-response plan, and a status namespace of their own for preview builds.
The plan (governance/incident-response.md, preliminary until counsel signs):
A1: every backup-plane administrator signs in with at least two factors, as NIST SP 800-209 (AC-SS-R5) asks for access to storage that holds mission-critical data.
A8(vi): the exemption file for restored data also holds the A4 severity assessment, with the PDPC notice's ข้อ 12 factors weighed, and the record of the check that the restored data is clean and usable. Marked for counsel.
After Playbook 7: ISO/IEC 27090:2026 is not bought now. It is bought, with ISO's separate licence for machine-learning use, when a real-world AI attack needs tracing against it. Until then, MITRE ATLAS (release v2026.10) and the OWASP lists carry the patterns of real attacks.
Previews:wrangler.jsonc binds STATUS in the previews block to a separate KV namespace, so a preview build never reads or writes production's status record. Cloudflare's own Previews pages say previews do not inherit production settings and that the scheduler does not call a preview's scheduled() handler, so the comments and the threat-model row that called this "not stated" now quote them. docs/DEPLOY.md and governance/inventory.md name both namespaces.
Clarified that preview deployments use separate status records, do not inherit production settings, and are not triggered by scheduled jobs.
Updated incident-response guidance with stronger authentication requirements for backup administrators, additional checks for restored data exemptions, and expanded AI-incident references.
Noted that the guidance remains preliminary pending legal approval.

Walkthrough
The PR configures a separate preview STATUS KV namespace and updates deployment and threat-model documentation. It also revises incident-response requirements for administrator MFA, AI-attack reference sources, and restored-data exemption records, with a changelog and sitemap hash update.
Changes: Preview Status Namespace
Layer / File(s)
Summary
Preview namespace binding and documentation wrangler.jsonc, docs/DEPLOY.md, THREAT_MODEL.md, governance/inventory.md
Preview configuration binds STATUS to a separate KV namespace. Deployment and threat-model documentation describes preview settings and scheduled-handler behavior. The asset inventory records the separate binding.
Incident-Response Requirements
Layer / File(s)
Summary
Incident-response policy and related records governance/incident-response.md, CHANGELOG.md, scripts/lib/sitemap-content.json
The policy adds a two-factor requirement for backup-plane administrators, details ISO/IEC 27090 licensing and interim references, and adds severity and clean-data checks for restored-data exemptions. The changelog records these updates and the preview status separation. The sitemap hash for the changelog entry changes.
These straightforward corrections should be made to avoid misleading operators about preview status, exemption decisions, and backup-plane MFA controls.

The status record is one object in a Workers KV namespace, bound as `STATUS` in `wrangler.jsonc`. The maintainer created the namespace through the Cloudflare API; its id is public: it grants no access by itself, since reading or writing the namespace needs an API token for the account. The scheduled run writes the record, and `/status.json` serves the stored result after the first scheduled run; until then it answers that no stored check result is available yet. `STATUS` is bound before the `ALERT_TO` secret is set, because without the record every failing run is mailed. `STATUS` is bound now and `ALERT_TO` is still unset, so no alert is sent.

Production's namespace is `kolwen-status` (id `5590bbcdccde496ba7e9c8ef1dd51f7b`); previews bind `STATUS` to a separate one, `kolwen-status-preview` (id `cdf17189811f4cd6a4311839c24cc019`), also created by the maintainer through the Cloudflare API, so a preview never reads or writes production's record. A preview could not write it anyway: Cloudflare's Previews resources page (https://developers.cloudflare.com/workers/previews/resources/, read 2026-10-30) says "Cron Triggers target production" and "the scheduler does not call a Preview's `scheduled()` handler today", and its configuration page (https://developers.cloudflare.com/workers/previews/configuration/) says "Previews do not inherit production settings".

Distinguish production and preview status updates.
The sentence says /status.json receives a result after the first scheduled run. That applies to production, not previews: the scheduler does not invoke preview handlers, so it cannot populate the preview namespace. (developers.cloudflare.com)
State that previews return no stored result unless their namespace is populated separately.

Disposition, from the maintainers: DEFERRED to the next change to this plan and its deploy notes, thread left open as the record. Right: "after the first scheduled run" holds for production only. Read once on this branch's preview at 18:59:48Z, /status.json answered {"checked":null,"message":"No stored check result is available yet."} while production held its 18:45:54Z record, as the Previews pages say. The sentence will say that a preview answers no stored result, because no scheduled run writes its namespace.

@HetCreep Acknowledged. The documentation change is deferred to the next update to the plan and deploy notes. This thread stays open as the record.
Your reported preview response is consistent with the production-only scheduled updates. Would you like a GitHub issue for the deferred documentation change?
**(v) Media holding line.** One paragraph, approved by the maintainer: that an incident is being handled, what is known, what people should do, where updates will appear. No speculation about cause or number.
**(vi) The exemption file (ข้อ 9).** Where Kolwen will claim that no notice to the Office is needed, it records at once the proof: for example data that cannot identify a person, or data not in a usable state because of adequate technical measures such as encryption, with the key intact. A decision not to notify is written down with its reason (EDPB para 125). For data an attacker encrypted and Kolwen restored from a copy, the claim is no risk, not the encryption example above, and its proof is the exfiltration check and the restore log (A3).

Use the existing legal risk threshold instead of “no risk.”
Line 66 defines the exemption threshold as a breach that is unlikely to risk people’s rights and freedoms. Line 221 says that restored attacker-encrypted data supports a claim of “no risk.” A clean restore and an exfiltration check provide evidence for the A4 and ข้อ 12 assessment, but they do not prove zero residual risk. Replace “the claim is no risk” with “the claim is that the breach is unlikely to risk people’s rights and freedoms.”

Disposition, from the maintainers: DEFERRED to the next change to this plan, thread left open. Right: A3 states the claim as a breach unlikely to risk people's rights and freedoms, and "no risk" in A8(vi) says more than that threshold. The line will use A3's words. The reading stays preliminary and for counsel.
