fix: clarify description and review scope

Changes

SKILL.md (issue2merge)

• Description: "review thread resolution" → "review comment tracking"
  • Watch agent only monitors and reports review threads, doesn't reply/resolve them. Description now matches actual behavior.

review-agent-prompt.md

• Task: "Review all uncommitted changes in {REPO_PATH}" → "Review all uncommitted changes on the working branch {BRANCH_NAME} (diff against main)"
  • Prevents Review agent from reviewing the entire repo. Only reviews branch diff against main.
