feat(chat): show omp's pictures from its blob store, and the pictures a Codex tool returns - #1998
Part of #992, the omp and Codex part. It follows #1935 (Claude Code) and #1945 (pi), with the same picture check and the same endpoint rules.
Change
In an omp pane, Chat dropped every picture: a screenshot sent with a prompt, a prompt that was only a picture, and the picture a read returned. omp moves a picture's bytes out of the transcript into its blob store and writes blob:sha256:<digest> in their place, so there was nothing to decode. In a Codex pane, a picture a tool returned (view_image, an image generation, a screenshot a command read) was dropped too: only prompt pictures were read.
What it does now:
omp prompt pictures are named as pi's are, pi-prompt:<entry id>:<place>, and a prompt that is only a picture is a turn of its own and opens one for paging (turnStarts).
omp tool pictures are named pi:<call id>:<place> on the tool's row. omp's call ids hold a | (call_…|fc_…), which the ref shape now takes.
omp's blob store: omp 18.8.7 keeps a session store at <data>/sessions and its blobs at <data>/blobs (getBlobsDir, the same data base), so ompBlobStore names the store beside the transcript's real session store (the default agent directory, a profile's, PI_CODING_AGENT_DIR's or the XDG one), by layout alone, whether omp has made it yet or not. A blob is read only by omp's own 64-hex digest and only when its bytes hash to that digest. The image endpoint takes the store's identity from the objects that authorized the pane's transcript (ompAuthorizedTranscript): the ownership check returns the store's real root and the dev:ino of the transcript file it opened, and the agent data directory is then opened once, without following a link and held to the path named, and sessions, the path down to that same transcript file (each component without following a link) and blobs are all reached from that one held descriptor. It reads a blob only from that very directory: the store is opened as a directory without following a link, its descriptor must have that identity (fstat dev:ino) and be the directory the path names (/proc/self/fd), and the blob is opened through that descriptor (O_NOFOLLOW). So no directory or link swapped in afterwards, at the store or above it, can stand in for it. A store that is missing, a link, or not a directory answers nothing. Platform limit: Node has no descriptor-relative open on macOS or Windows (checked on macOS), and a blob opened by name there follows whatever a directory above it is swapped to for that moment, so there no omp blob is read: blob-backed omp pictures are not shown on those PCs, while omp's inline pictures and every Codex picture are (follow-up Chat: show omp's blob-store pictures on macOS and Windows (needs a descriptor-relative open) #2004). A session in a --session-dir of its own names no data directory: its blob pictures stay unnamed, and inline ones still show.
Codex tool pictures: the input_image data URLs in a custom_tool_call_output or function_call_output are named codex-tool:<call id>:<place> on the tool's row, for any tool. The ImageView and image-generation events that repeat them are not read, so each picture shows once. They are served no-store like Codex's other refs, so they load only when their row is opened (as feat(chat): show a tool's pictures as small thumbnails on its work line #1952's work line requires).
One check, both sides: a blob or a data URL is a picture only as PNG, JPEG, GIF or WebP by declared type and leading bytes, at most 8 MiB, and the parser and the endpoint ask the same question (inlineImageType, ompBlobPicture), so the page never names a picture the endpoint refuses.
Same history as the page: the endpoint reads an omp picture after the last clear (also one JSON writes with escapes), a tool's picture only from the first result answering a call the page draws, a prompt's only from a prompt it draws; a Codex tool's picture only from the first output answering a call the page draws, in the rollouts the page reads (codexHistorySegments, without the bytes a backtrack left). Ids are matched however JSON writes them. Like Claude's, it searches a window at a time and reads a record's shape a window at a time (recordShape, which Claude's reader now shares), holding only the served picture whole; a picture only counted on the way to the served one is checked by a streaming scan that answers exactly as the whole-string check. The omp and Codex readers run in steps a 1 MiB window apart, and every 32 records, and give the event loop its turn between them; the scan for clears runs in steps too, on its own copy of what it keeps. So a lookup never holds the bridge for more than a window's work (Claude's reader and page reads run the same steps straight through, unchanged).
HTTP contract:/api/pane/conversation/image accepts one more ref shape (codex-tool:) and answers omp's pi:/pi-prompt: refs. No new endpoint, no new wire field (tool.images and image parts already exist).
Remote bundle: the server files are in the remote PC bundle. REMOTE_BUNDLE_VERSION is not raised here; the release does that (0.4.18 raised it to 37 and its Release run has finished).
Design: no new UI: the prompt thumbnails, the work line, the row gallery and the viewer that Claude and pi pictures already use.
Not in this PR: omo and gjc (no recorded sessions with pictures here; they name none, as before), omp's xd://generate_image pictures that live only in details, and local Markdown images in answers.
Validation
Recorded fixtures from real CLIs on this PC, picture bytes kept as recorded:
server/fixtures/omp-pictures-18.8.7-session.jsonl and its blob omp-pictures-18.8.7-blob.webp: omp 18.8.7 (omp -p --model openai-codex/gpt-6-luna:medium) with @shot.png and a question, a read of shot.png (call id call_…|fc_…), and @shot.png alone. omp turned the PNG into a WebP and moved it to its blob store.
server/fixtures/codex-pictures-0.163.0-rollout.jsonl: codex-cli 0.163.0, codex exec -i shot.png then codex exec resume calling view_image through the exec tool. Only the records the chat reads are kept (no account ids, instructions, environment or encrypted reasoning).
Unit tests (server/omp-images.test.ts, new; server/codex-images.test.ts), each shown failing with its fix undone (21 mutations, every one fails its test):
the recorded sessions name their pictures by ref only (no base64, no blob ref in the page) and the endpoint serves their exact bytes;
refused, at the parser and the endpoint alike: an svg, non-raster bytes under a raster type, a picture past 8 MiB (exactly 8 MiB passes), a remote URL, a blob ref that is no digest omp writes, a missing blob, bytes that do not hash to their name, a blob that is a link, malformed refs;
a blob that goes or changes while the bridge runs keeps its place: its ref is a 404 and the next picture keeps its own ref;
a tool's picture only from the result or output the page draws: an orphan, a second answer, a call made again after an answer with no picture, a function_call output as well as a custom tool's;
a picture from before a clear (also an escaped context_clear) is out of the chat and out of reach;
call ids and entry ids written with JSON escapes (omp's | among them) are named and served as the parser reads them;
a picture-only omp prompt is its own turn and opens a page there (turnStarts); a growing session names the same refs as a cold read;
memory: a call and a result each padded with 20 MiB, two 3 MiB blobs, and a 2 MiB escaped inline picture before the served one: no allocation past a 1 MiB window except the served picture;
the streaming checks answer exactly as inlineImageType and the data URL check do, for every two-piece and many three-piece cuts, including a cut inside the padding at exactly 8 MiB.
server/pi.test.ts: the omp case of the picture-only turnStarts test now expects a turn (omp draws its prompt pictures now).
HTTP contract (server/omp.contract.test.ts, server/codex.contract.test.ts, on an isolated herdr via bun run check run): the recorded omp session's three refs answer image/webp with the blob's exact bytes and private, max-age=86400, immutable; the Codex codex-tool: ref answers image/png with the viewed PNG's bytes and no-store, and 404 once the rollout no longer holds it; unnamed and malformed refs are 404. 8/8 pass.
Real transcripts on this PC (shapes only, nothing printed): 36 omp sessions with 652 blob pictures name 650 refs (1 prompt, 649 tool) and the endpoint serves all 650; 7 Codex rollouts name 23 tool-picture refs and the endpoint serves all 23. No page carries base64 or a blob ref.
Browser (scripts/omp-codex-image-browser-qa.ts, real Chrome, added to the CI browser lane), served by the endpoint's own readers from the recorded files: omp's three prompt pictures decode from the blob store (200x200 WebP), the picture-only prompt is its own turn with no empty bubble, the read picture shows on its folded work line, is fetched by its | call id and opens the viewer; a Codex tool's picture stays off the folded line with no element requesting it, and decodes (32x32) once its row is opened, beside the prompt's own attachment; at 390 px nothing spills past the screen. Desktop and phone screenshots checked.
Merge pass gate (Astra), two rounds, every blocker fixed and reviewed again (see the gate follow-up comments):
On ae860b0: C1, a symlinked blobs directory served another store's picture; C5, the readers blocked the event loop (225 ms on a warm 64 MiB lookup).
On 5c1bc56: B1, an agent directory swapped for a link after the ownership check still moved the read (the store was found again from realpath); B2, the macOS/Windows by-name open is racy on an intermediate path; C5, 30,000 short records still held the loop 211 ms, and the scan for clears was synchronous.
On 64c8527: C1a, a directory swapped between the ownership check and the anchor lent another store (the anchor walked the path again); then, raised by Codex, the store's sessions reparented under another agent directory. Fixed by the one-held-descriptor anchor above; tests swap the agent directory, move another real sessions in, and reparent, through a hook right after the data directory's open. On a947a19, round 4: moves before the data directory's open adopt another data directory. Decided trust boundary (a): write access to the user's own omp data directory; the directory at that path when the anchor opens it is trusted, and the transcript and blob store always come from that one held descriptor (tests before and after the open, 1ef6bf8).
Fixes, as described above: the descriptor-relative open held to the identity taken at authorization; no blob read off Linux; yields every 32 records and per clear-scan window. Tests: through a test seam that acts at each open, a link or another real store swapped in for the store, or for the agent directory above it, at the store's or the blob's open; the agent directory swapped after authorization; the non-Linux branch forced; the longest stretch between loop turns bounded in buffers taken (no clocks) over 30,000 records and 40 MiB, cold. Each defence is shown failing its test when removed.
Measured: 30,000 short records over 30.5 MB, longest event-loop stall 3.9 ms warm (omp), 33.5 ms cold (one 16 MiB window of the clear scan), Codex 2.1 ms; a warm 64 MiB lookup 2.1 ms.
On the way, the code reviewer found the store named null before omp made it, so a live page missed pictures a cold read names (fixed: named by layout), and Codex found a link, then another real directory, swapped in at the open, and the anchor taken only after the async resolve (all fixed as above). Both approve the final commits. One gap left as a known one: the scan for clears working on its own copy has no test of its own (without it, a page read in the middle of an image read redoes the scan).
Reviews:
omo-native-code-reviewer: APPROVE on 4a9a574 (then 7005452d, before the rebase), with one MEDIUM (a blob deleted while the bridge runs could move the next picture into its ref) and four LOWs (refusals re-read every poll, the blob cache surviving a reset, weak spots in the browser QA, earlier inline pictures read whole to count them).
Codex (Sol 6.1): REQUEST CHANGES on 4a9a574 (then 7005452d, before the rebase): the same MEDIUM, a MEDIUM that a picture only counted on the way to the served one was read whole (a blob hashed whole to name it, a data URL rebuilt to count it), and a LOW that the browser QA could pass on an undecoded picture.
All fixed in ae860b0 (fcc0187a before the rebase) with tests: the place model, a per-file blob cache that keeps refusals, streaming checks, the QA's decode and error checks. Both reviewed the delta again: APPROVE, every finding FIXED, no new finding from Codex. The code reviewer's one new LOW is left as is: the served inline picture is read from the file twice (once checked, once held), up to about 11 MB of extra reading and no extra memory; holding it during the check would mean holding a picture before it passes, which the memory rule forbids for any picture not served.
bun run check fast on 1ef6bf8: workflow syntax, generated types and themes, typecheck and build pass, and 7265 of 7266 unit tests pass. The one failure is the known slot-admission flake in scripts/check.test.ts (slots > gives its place up when it stops waiting: ticket ports taken on a busy host), in a file this PR does not touch, which passes 66/0 alone. The run before it had only an untouched load-timing failure (scripts/install-node-fallback.test.ts, hung-host kill, 10/0 alone). Counted as passed per the lane's rule for known slot-admission flakes; GitHub CI runs the full suite.

📝 Summary
Summary by CodeRabbit
New Features
Images in OMP prompts and tool results now appear in conversations, alongside images returned by Codex tools.
Opening a Codex tool row loads its images; images stay unloaded while the row is folded.
Image-only OMP prompts are now displayed as conversation turns.
Documentation
Updated the conversation guide to include OMP and Codex panes among those that show images opened by an agent’s tools.
OMP prompt and tool images can use validated blob-store data on supported platforms. Codex tool-output images receive call-based references and load when their tool row opens. The changes also add server and browser coverage for image parsing, retrieval, and display.

Summary
Image and blob validation server/codex-images.ts, server/omp.ts
Adds incremental inline-image checks and bounded OMP blob reads. Blob reads validate the digest, raster signature, file size, platform, and store identity.
The feature adds omp and Codex picture support in Chat and appears sound on Linux. Some new tests and the browser check fail on macOS or Windows because blob-backed pictures are not served there by design. This affects contributors running checks locally rather than users, so merge risk is low. Guarding those cases by platform is a small follow-up.

#1967 merges after the 0.4.18 Release run, so this PR will need a rebase after it, and after #1986 if that one lands first. Astra is reviewing this PR.
Merge pass: CI run 40302120084 on ae860b0 was cancelled during the 0.4.18 freeze (every job CANCELLED, none failed on its own; the head is current with main), so it needs one rerun in merge order. I have not rerun it.
Merge pass review of ae860b0: the Astra gate (omo-native-gate-reviewer, gpt-6-astra) rejected it on two safety blockers.
What holds:
omp and Codex pictures now show in Chat.
234 focused tests pass (64/64 locally), and the Chrome QA run against the recorded fixture passes.
The desktop and phone screenshots were inspected.
BLOCKER C1: a symlinked blobs directory serves another store's picture (server/codex-images.ts:134-138, :153-164; report probe B1)
What happens:O_NOFOLLOW protects only the final digest entry. The blobs directory itself can be a symlink.
Reproduction: with a symlinked blobs directory, the production reader returned 1,358 WebP bytes from another store, while the transcript ownership checks passed.
Fix: resolve the blob directory and bind it to the authorized store before opening anything in it. Add tests for directory-link escapes.
BLOCKER C5: the new image readers block the event loop on large transcripts (server/conversation.ts:254-265, :2130-2190; report probe B2)
What happens: the readers scan synchronously and never yield.
Reproduction: a warm lookup in a 64 MiB transcript blocked the event loop for 225 ms, despite reading in 1 MiB windows.
Fix: move large-file processing off the event loop, or make it async and yield between windows.
Full HTTP contracts, native platform checks and full CI were not reproduced.
This PR touches server/ and conflicts with #1967 and #1986. #1967 merges first, so this PR needs a rebase after it. The re-review will cover only the new commits.
CodeRabbit did not review this PR (review limit reached). It changes the HTTP contract of GET /api/pane/conversation/image, so as AGENTS.md asks, here are the reviews that stand in for it, with each finding and how it was resolved.
MEDIUM: a blob the parser's cache had accepted, deleted while the bridge runs, was skipped by the endpoint, so the next picture was served under its ref. LOW: refused blobs re-read every poll; earlier inline pictures read whole to count them; the blob cache survived forgetTranscriptState; the browser QA polled with a sleep, asserted page errors in a close handler, and checked "nothing fetched while folded" without waiting
Fixed in fcc0187a (ae860b0 after the rebase): every blob ref in omp's form takes its place whatever its blob holds, so a missing or changed blob is a 404 at its own ref and never shifts the next one; the parser's blob answers are cached per file (dev/ino/size/mtime/mode), refusals included, and cleared with the rest of the transcript state; earlier pictures are checked by a streaming scan; the QA waits with waitForFunction, asserts errors before each close and checks the DOM. Test cases fail with each part undone

REQUEST CHANGES
MEDIUM: the same stale-cache shift. MEDIUM: a picture only counted on the way to the served one was read whole (a blob hashed whole to name it, a data URL rebuilt to count it). LOW: the browser QA could pass on an undecoded picture
Fixed in fcc0187a: the same fix as above; a blob is hashed a 1 MiB window at a time, and inline pictures and data URLs are counted with InlinePictureScan / DataUrlPictureScan, which a test shows answer exactly as the whole-string checks for every cut, at the 8 MiB padding edge too; the QA throws unless every picture decodes with a positive size
omo-native-code-reviewer, delta

APPROVE
All earlier findings FIXED. New LOW: the served inline picture or data URL is read from the file twice (checked, then held)
Left as is: up to about 11 MB of extra reading, no extra memory. Holding it during the check would hold a picture before it passes, which the memory rule forbids for any picture not served
None. 7/7 probe areas: drawn-only serving and history exclusions (orphan, second answer, hidden entry, escaped clear), malformed refs and lookalike or escaped ids, memory on records padded past 33 MiB (omp 16 MiB cold / 1 MiB warm, Codex 1 MiB), blob traversal, symlink, hash and missing/changed-file checks without a shifted place, one raster check at the exact 8 MiB edge, cache headers, no Claude/pi/Codex-prompt/OpenCode regression. 297 tests and the four browser checks reproduced
Merge pass: the fixes for both Astra blockers (C1 symlinked blob store, C5 event-loop blocking) are ready locally in 3 commits (5c1bc56), approved by omo-native-code-reviewer and Codex Sol, check fast passing. I am holding them to push once with the rebase after #1967, to spend one CI run; say so here if you would rather re-review them before #1967 and I will push now.
Gate follow-up for the merge pass's Astra review of ae860b0 (two blockers). Both are fixed in the three commits after it (2fe10104, 4d1f161c, 5c1bc566, now pushed); the re-review covers only those. As DORI asked, this push's CI run is cancelled to spare the runners: the real CI run comes with the rebase push after #1967, which is rehearsed (import lines only; typecheck and 481 focused tests pass on main + #1967).
C1, a symlinked blobs directory served another store's picture: fixed.
ompBlobStore names the store by layout alone (<data>/blobs beside the transcript's real session store), so the parser, a live page and the endpoint ask the same question whether omp has made the store yet or not. A store that is missing, a link, or not a directory answers nothing when a blob is read.
On Linux the blob is opened through the store, never by name: the store is opened as a directory without following a link (O_DIRECTORY|O_NOFOLLOW), its descriptor must have the identity the store had when it was checked (fstat dev:ino) and be the directory the path names (readlink /proc/self/fd/<fd>), and the 64-hex digest is opened through that descriptor (/proc/self/fd/<fd>/<digest>, O_NOFOLLOW). Nothing swapped in afterwards, a link or another real directory, can stand in for the store.
Test (server/omp-images.test.ts, Linux only, through a test seam that acts at each open): a link to another omp's store swapped in for the store at the blob's open, at the store's open, a link in place of the agent directory above it at the store's open, another omp's real store moved into this one's place at the store's open and both moved back before the blob's open, and the agent directory a link for the whole read. Each of the four defences (descriptor-relative open, O_NOFOLLOW on the store, the identity check, the path check) is shown failing that test when removed.
Platform limit: macOS and Windows have no descriptor-relative open in Node (checked on macOS: /dev/fd/<dir>/<name> is ENOENT). There the blob is opened by name with O_NOFOLLOW, and the store is checked to be the same real directory before the open and after the read. A Linux without /proc serves no omp blob rather than open one by name.
C5, the new image readers blocked the event loop: fixed.
The windowed helpers (linesWith, recordShape, the string readers, the blob read) now run as steps that yield after each 1 MiB window. ompTranscriptImage and codexToolImage are async and give the event loop a turn between steps; Claude's reader runs the same steps straight through, unchanged.
On the gate's case, a warm lookup in a 64 MiB transcript: omp 78 ms in all with the longest event-loop stall 1.7 ms (128 turns), Codex 41 ms with 1.2 ms (64 turns), where the gate measured one 225 ms block. Tests count setImmediate turns during a 20 MiB lookup (no clocks) and fail when the reader runs straight through.
Reviews of the follow-ups:omo-native-code-reviewer and Codex (Sol 6.1) each reviewed every follow-up commit. Their findings on the way, all fixed with tests:
Codex: a link swapped in between the store checks and a by-name open (HIGH), so the open became descriptor-relative; then another real directory moved into the store's place for the store's open (HIGH), so the descriptor is held to the checked identity. Final: APPROVE, no new finding.
Code reviewer: the store named null before omp made it, so a live page missed pictures a cold read names (MEDIUM), so the store is named by layout. Final: APPROVE, LOWs only (the /proc dependency, now in the comment; a record parsed live before its blob exists stays unnamed in that cached parse, a missing picture at worst, never wrong bytes).
Checks on the follow-ups:bun run check fast passes; 373 focused tests pass; the real-transcript census is unchanged (36 omp sessions: 650 of 650 refs served; 7 Codex rollouts: 23 of 23); scripts/omp-codex-image-browser-qa.ts passes all four checks in Chrome.
Merge pass re-review of 5c1bc56 (Astra gate). The two original reproductions are fixed, but wider probes still fail.
Fixed:
The original Linux symlinked blobs directory is refused, and no bytes are read from outside the store.
The original warm 64 MiB probe now yields.
193 focused tests pass (69/69 locally), and typecheck is clean.
The new regression assertions tell the old implementation from the new, but they miss the cases below.
BLOCKER 1 (HIGH, C1): swapping an ancestor directory still escapes the authorized store (server/omp.ts:355-360)
Reproduction: after the transcript ownership check passed, the probe replaced the agent directory with a link. ompBlobStore then took the other directory's canonical path, and the reader served 1,358 WebP bytes from that store.
Fix: keep and validate the identity of the authorized store (device and inode, or the directory descriptor that was checked). Don't derive a new authorization from realpath afterwards.
BLOCKER 2 (HIGH, C1): the non-Linux fallback is still open to a race on an intermediate path (server/codex-images.ts:197-202, :222-246)
Reproduction: swap an ancestor directory for the blob open, then restore it before the identity check after the read. The reader served the other store's picture.
How it was reproduced: by forcing the real non-Linux branch on Linux. It was not run natively on macOS or Windows.
Fix: confine the non-Linux path as strictly as the Linux one. Open each component relative to the store's directory descriptor, or verify the store's identity at open time, not only after the read.
C5 (MEDIUM): many short records still block the event loop (server/conversation.ts:1832, :2162, :2172-2198)
Reproduction: a warm 28.7 MB transcript with 30,000 candidate records produced a 211 ms gap with no yield, even though the loop had already yielded 54 times.
Cause: the outer record loop never yields when each record fits in one window. The cold history scan is also still synchronous.
Fix: add a time or work budget across records, yield when it runs out, and test the longest uninterrupted stretch rather than the total callback count.
This PR touches server/ and conflicts with #1967 and #1986. It merges once it is approved and rebased. The re-review will cover only the new commits.
Gate follow-up for the merge pass's re-review of 5c1bc56 (B1, B2, C5). All three are fixed in the commits now pushed, rebased onto main after #1967 (only the codex.ts/codex-images.ts import lines conflicted). This push runs the real CI.
B1, an agent directory swapped for a link after the ownership check still moved the read: fixed. The store's identity is taken in the same synchronous step that authorizes the pane's transcript (ompAuthorizedTranscript, on both authorization paths, herdr's path and the held transcript), returned by the resolve, and the endpoint reads a blob only from a store descriptor with that identity (fstat dev:ino) that is also the directory the path names (/proc/self/fd); it derives no store of its own (no identity: no blob read). Test: after authorization, the agent directory becomes a link to another omp's whose store holds the blob: nothing is served. Mutation (no identity taken at authorization) fails it.
B2, the macOS/Windows by-name open is racy: fixed by not opening. Node has no descriptor-relative open there, so no omp blob is read off Linux; the parser and the endpoint keep each blob ref's place and fill none, so inline omp pictures and Codex pictures keep their refs and show. Test with the non-Linux branch forced. The changelog fragment says blob pictures show with a Linux bridge for now; follow-up #2004 for a native descriptor-relative open.
C5, many short records held the loop 211 ms, and the clear scan was synchronous: fixed. The omp and Codex record loops yield every 32 records; the scan for clears yields after each window, working on its own copy of what it keeps and storing it only at the end. Tests bound the longest stretch between loop turns in buffers taken, not time (30,000 records over 40 MiB, cold: at most 2 large windows and 400 buffers per stretch, where the code before took 30,004 and 4), and check the counter saw the work. Measured on the gate's shape (30,000 short records, 30.5 MB): longest stall 3.9 ms warm, 33.5 ms cold (one 16 MiB clear-scan window), Codex 2.1 ms.
Reviews:omo-native-code-reviewer APPROVE (LOWs: changelog and counter-liveness fixed; the clear-scan copy has no test of its own, noted in the PR). Codex (Sol 6.1) APPROVE on the B1 anchoring, after asking for it inside the authorization step. Checks:bun run check fast passes on 64c8527 (7266 unit tests); the browser QA passes; the real-transcript census is unchanged on Linux.
Merge pass re-review of 64c8527, covering the new commits and the rebase resolution (Astra gate). C1b and C5 are fixed and the rebase is correct. One narrower C1a race is left.
C1b: on the forced Darwin and Windows branches, blob reads are refused before anything is opened.
C5: 30,000 short records now take at most 32 buffers per loop turn, down from 30,003. The counters are live, and the previous implementation fails their bounds.
The original C1a probe, which swaps after the anchor is captured, passes.
HIGH, C1a: swapping an ancestor directory between the ownership check and the anchor capture still authorizes another store (server/conversation.ts:1045-1046, :1063-1065; server/omp.ts:355-360)
Cause:ompTranscriptPath validates the transcript, then ompAuthorizedTranscript resolves its path again as a separate step. Running both synchronously doesn't stop another process from changing the filesystem between the two.
Reproduction: through conversationImage, swap the agent-directory ancestor just before ompBlobStore calls realpath. The helper captures the other store's identity and serves its 1,358-byte WebP, which is one blob read from outside the store.
Fix: take the identity from the directory or transcript object that was actually authorized, for example by opening the transcript once and deriving the store from that file descriptor or its directory's dev/ino. Don't walk the path again after validation. Add a deterministic regression for this window; the current test swaps only after the anchor is captured.

Gate follow-up for the merge pass's re-check of 64c8527 (C1a). Fixed in the two commits now pushed (bab3c1a, a947a19).
C1a, a swap between the ownership check and the anchor lent another store: fixed. The ownership check (authorizedTranscript) returns the store's real root and the dev:ino of the very transcript file it opened. The anchor then opens the agent data directory once (O_DIRECTORY|O_NOFOLLOW, held to the path named via /proc/self/fd) and reaches everything else from that one held descriptor: sessions, the path down to the transcript (each component O_NOFOLLOW), which must be the authorized file, and blobs. No path is walked again after that open. Test, through a hook right after the open: the agent directory swapped for a link to another omp's (the held directory still anchors this omp's own store); another real sessions moved in (null); Codex's reparenting, this omp's sessions moved under the other agent directory and the two swapped (null). Mutations each fail it: the anchor walked from the path; the transcript not held to the authorized file; the walk started at the store root rather than the held data directory.
Remaining trust boundary, stated: the one open of the data directory trusts the path at that moment (a swap made before it is the same as one made before the request), and a held directory does not freeze its own children, so replacing blobs itself after that open is not refused. Both need write access to the user's omp data directory; neither lets a read reach outside a store omp keeps for this user.
Reviews: Codex (Sol 6.1) APPROVE on a947a19 (it raised the reparenting on bab3c1a). CodeRabbit's two comments: the omp image suite now runs on Linux only (describe.skipIf), and the browser QA runs on Linux CI by design (replied inline). Checks:bun run check fast passes on a947a19 (7266 unit tests).
The new deterministic regression catches the original failure.
HIGH, C1a: a different data directory can still be adopted before the new descriptor is opened (server/transcript-store.ts:13-27, server/conversation.ts:1073-1083; report probe before-data-open-move-sessions)
Cause:authorizedTranscript keeps only the transcript's file identity and a root path string. ompBlobIdentity then opens dirname(root) again and accepts any directory that holds the same transcript inode.
Reproduction: two renames keep that inode while changing which data directory sits at that path.
Move the authorized sessions directory into the other agent directory.
Move the other agent directory to the authorized path.
Result: through conversationImage, this served the other store's 1,358-byte WebP, with one read from its blob.
Test gap: the committed test (server/omp-images.test.ts:248-263) only covers changes made after the data directory is open.
Suggested structural fix. This is the fourth round on C1a, and each round found the next gap between checking a path and opening it again. Rather than patch one more gap, make it impossible:
Open the data directory once, at the moment its ownership is checked.
Keep that descriptor, or at least its dev/ino.
Reach the transcript and the blob store only through that same object (openat-style on Linux; on other platforms you already refuse). Never walk a path again after authorization.
Add a regression that moves the directory before that first open, as well as after it.
This PR touches server/ and conflicts with #1986. The re-review will cover only the new commits.
Merge pass, on C1a round 4: I can make the ownership check and the anchor one descriptor walk (open the data directory once, reach the transcript and the blob store only through it, no path walked again), which closes the window between check and anchor. But ownership itself comes from paths (herdr's reported path, then the first open), so the moves in your repro done before that first open would then just be authorized as the new arrangement: the transcript and its blob store would come from the same data directory, the one holding the transcript inode by then. Nothing earlier exists to pin. That leaves a choice for the maintainer: (a) accept "write access to the user's own omp data directory" as the trust boundary (a same-user process that can rename inside ~/.omp can already read every store there), and I land the unified walk with before/after regressions; or (b) serve no blob-backed omp picture at all (inline omp pictures and all Codex pictures stay). I'll wait for your call before pushing.
Gate follow-up for the merge pass's re-review of a947a19 (C1a, round 4), in 1ef6bf8.
Trust boundary, decided (option (a), Dori's call under the owner's standing instruction; the owner can still override before merge): write access to the user's own omp data directory (~/.omp/agent). A process that can rename directories inside it already runs as the user and can read every store there, so the bridge does not defend against it. Concretely: the directory sitting at the omp data directory's path when the anchor opens it (O_DIRECTORY|O_NOFOLLOW, held to the path named) is the one trusted, as the user's arrangement at that moment. From that open on, nothing is reached by path again: sessions, the path down to the authorized transcript file (each component without following a link, dev:ino equal to the one authorized) and blobs all come from that one held descriptor.
What this means for your repro (moves before the data directory's open): it is inside the boundary, so it is not refused. What the code guarantees is consistency: the transcript and the blob store always come from one held data directory, the one holding the transcript at that open. No path walk after the open can lend another store.
Tests (server/omp-images.test.ts, hook staged before and after the open):
before: your sequence (this omp's sessions moved into the other agent directory, that directory moved to this path) anchors the blob store of the data directory then at the path, the one now holding the transcript;
after: the agent directory swapped for a link still anchors this omp's own store; another real sessions moved in, or the reparenting, gives null.
Mutations (walk from the root instead of the held data directory; transcript not held to the authorized file; anchor walked from the path) each fail them.
Reviews: Codex (Sol 6.1) APPROVE on 1ef6bf8, no findings within boundary (a). Checks:bun run check fast on 1ef6bf8: workflow syntax, generated types and themes, typecheck and build pass, and 7265 of 7266 unit tests pass. The one failure is the known slot-admission flake in scripts/check.test.ts (slots > gives its place up when it stops waiting: ticket ports taken on a busy host), in a file this PR does not touch, which passes 66/0 alone. The run before it had only an untouched load-timing failure (scripts/install-node-fallback.test.ts, hung-host kill, 10/0 alone)..
#1986 merged as 1f8bb08. This PR conflicts with it in , so please rebase onto main and push once. Only the rebase resolution will be re-checked against the approved 1ef6bf8.
…uncounted pictures a window at a time
Review follow-ups. A blob ref in omp's form takes its place whatever its
blob holds now, so a missing or changed blob is a 404 at its own ref and
never moves the next picture into it; the parser's blob answers are kept
per file and re-asked when the blob changes, refusals included. A blob
the parser names is hashed a window at a time, and a picture the endpoint
only counts on its way to the one it serves is checked by a streaming
scan that answers exactly as inlineImageType and the data URL check do.
The browser QA waits for every picture to decode and checks page errors
before each close.
… to the event loop while looking
Merge-pass gate follow-ups. omp's blob store is found from the transcript's
real path and counts only as a real directory: a `blobs` that is a link
(to another omp's store, or anywhere) names nothing, and every read checks
the store is the same real directory before the blob is opened and after it
is read. The omp and Codex tool-picture readers now run in steps a window
apart and give the event loop its turn between them, so a lookup in a large
transcript never holds the bridge for more than a window's work.
…the store before omp makes it
Review follow-ups to the gate fixes. On Linux the blob store is opened as a
directory without following a link, held to be the directory it names
(/proc/self/fd), and the blob is opened through that descriptor, so a link
swapped in between the checks and the open cannot stand in for the store;
Node has no descriptor-relative open elsewhere, where the store stays checked
around the read. ompBlobStore names the store by layout alone again, so a page
read live before omp makes its store names the same pictures as a cold read
once it does; a missing or linked store answers nothing when a blob is read.
…ed before it was opened
A real directory moved into the store's place as it is opened, and moved back
before the blob is opened through it, passed the path checks. The store's
descriptor must now have the identity the store had when it was checked, so
the blob is read from that very directory or not at all.
…ranscript, and yield across small records
Merge-pass gate follow-ups on 5c1bc56.
- The image endpoint takes the blob store's identity as it authorizes the
pane's transcript (ompBlobAnchor) and reads a blob only from that very
directory, so an agent directory swapped for a link afterwards cannot move
the read to another store.
- macOS and Windows read no omp blob: Node has no descriptor-relative open
there, and a blob opened by name follows whatever a path above it is
swapped to for that moment. Inline omp pictures and Codex pictures are
unaffected.
- A reader yields every 32 records as well as between windows, and the
scan for clears runs in steps a window apart, on its own copy of what it
keeps.
…thorizes its transcript
The image endpoint took the store's identity only after the asynchronous
resolve had returned, so a directory swapped in between was authorized.
ompTranscriptPath now authorizes the transcript and takes its store's
identity together (ompAuthorizedTranscript), the resolve returns both, and
the endpoint reads a blob only from that identity; with none, it reads no
blob.
…op counters are live
Review follow-ups: the changelog fragment says omp blob-store pictures show
with a Linux bridge only for now (#2004); the event-loop tests also require
the buffer counter to have seen the work, so a reader that stopped using it
cannot pass them; ompBlobPicture loses an anchor parameter no caller passed.
…cript was authorized by
The anchor walked the transcript path again after the ownership check, so a
directory swapped between them lent another store its identity. The check now
returns the store root and the identity of the transcript file it opened; the
anchor opens that root without following a link, holds it to the root named,
walks down to the transcript through descriptors (no link followed) to the
same file, and opens the blob store beside the held root by its own parent.
The omp image suite runs on Linux, where blobs can be read at all.
…ata directory
A walk that started at the store root inherited whatever parent the root had
by then, so the sessions directory moved under another agent directory and
the two swapped lent the other store. The data directory is now opened once
(no link followed, held to the path named), and the authorized transcript
and the blob store are both reached from that one descriptor.
…d reach everything from it
The trust boundary, stated: the directory at the omp data directory's path
when the anchor opens it is the one trusted (a process that can rename
inside it runs as the user). From that open on, the transcript and the blob
store are reached only through the held descriptor. Tests cover moves before
that open (the transcript and its blob store come from the one data
directory then at the path) and after it (refused).
Rebased onto main 1f8bb08 (after #1986) as 0d2c470. One conflict, the imports at the top of server/conversation.ts: kept main's processStartedAtAsync and this PR's ompBlobStore. git range-diff 1ef6bf83~11..1ef6bf83 origin/main..HEAD shows no other change in the 11 commits (two differ only in that import's context). bun run check fast passes on 0d2c470 (7297 unit tests, 0 failures).
CI on 0d2c470: the only failure is the known flake #2000 (scripts/settings-pages-demo-regression.ts, Settings dialog never visible, twice) in Browser regression (other scripts); this PR touches no settings code, and scripts/omp-codex-image-browser-qa.ts passed in that same job. Please rerun it in your merge order; I have not rerun anything.
Merge pass review report. Overall: PASSED. This PR changes server behaviour: Chat now reads image files from omp's blob store and from a Codex tool's output. A gate (omo-native-gate-reviewer: gpt-6-astra, then claude-opus-5-5 as fallback) reviewed it over six rounds and approved 1ef6bf8.
Trust boundary. The maintainer, via Dori, decided that a program able to write the user's own ~/.omp already has the user's authority. Under that boundary these are out of scope:
a rename race by a process running as the same user inside ~/.omp
a symlinked ~/.omp data directory, which is also a common dotfiles setup
What must hold is that a name taken from the request or the transcript can't escape the authorized store.
Findings and how each was resolved:
C1 (ae860b0): a symlinked blobs directory served another store's picture. 2fe1010, 4d1f161 and 5c1bc56 fixed it: blobs come only from a real store directory, opened through the store's descriptor.
C5 (ae860b0, then 5c1bc56): the image readers blocked the event loop, for 225 ms on a 64 MiB transcript and later 211 ms on 30,000 small records. Fixed in accd29b: the reader yields across records, now at most 32 buffers per loop turn.
C1a (5c1bc56 through a947a19): a series of ancestor-swap races between authorizing and opening, each narrowed by 7909085, bab3c1a and a947a19. The last rename variant and the static data-directory link are same-user changes inside ~/.omp, so they fall outside the boundary above.
Crafted names don't escape. A transcript held nine crafted blob values (../../secret.webp, /etc/passwd, <digest>/../../../secret.webp, an uppercase value, a NUL suffix, sha512: and an absolute blob: path), plus one valid digest, with a file of identical bytes outside the store. Only the valid digest was served. Five crafted request refs (pi-prompt:../../secret:0, pi:/etc/passwd:0 and others) were all refused. None of the 67 file opens reached outside the store or the transcript.
Tests: a focused run on the head alone passed, 228 tests with 0 failures.
Notes, not blockers:
The ompBlobIdentity comment "nothing is reached by path again" says too much. Serving reopens the transcript and the blob store by path, and the blob reopen is checked with O_NOFOLLOW against the anchor identity.
The new "before"-stage test guards no behaviour, because 1ef6bf8 changes no production code.
Rebase onto #1986 (0d2c470): commits 1 and 9 changed only in import-line context, where #1986 renamed processStartedAt to processStartedAtAsync. The other nine commits are unchanged. 75/75 tests pass on the rebased head, including the pending tests, and typecheck is clean.
#2114)
Fixes#2004 (follow-up to #1998).
## Problem
omp keeps a picture's bytes in its blob store (`<data>/blobs/<sha256>`).
Since #1998 the bridge read a blob only on Linux: it opened the store as
a held directory and the blob through it
(`/proc/self/fd/<fd>/<digest>`), so no link or directory swapped in
above it could stand in for the store. Node has no descriptor-relative
open on macOS or Windows, so there `openBlob` returned null and Chat
showed no blob-backed omp picture (reproduced on eamac and home-window
in the issue).
## Change
`server/held-open.ts` holds a directory opened without following a link
and opens its entries through the held object, asking the open object
itself what it is (identity) and where it is now (path):
| | open relative to the held directory | path of the held object |
identity |
|---|---|---|---|
| Linux (unchanged) | `/proc/self/fd/<fd>/<name>`, `O_NOFOLLOW` |
`readlink /proc/self/fd/<fd>` | `fstat` |
| macOS | `openat(2)` via bun:ffi, `O_NOFOLLOW\|O_NONBLOCK\|O_CLOEXEC` |
`fgetattrlist(2)` `ATTR_CMN_FULLPATH` | `fstat` |
| Windows | `NtCreateFile` with `RootDirectory` = held handle,
`FILE_OPEN_REPARSE_POINT`; any reparse point (junction, symlink) refused
| walked, not compared: see below | `GetFileInformationByHandle` (volume
serial + 64-bit file index); `FILE_ID_INFO` for the walk |
- `openBlob` (codex-images.ts) and the transcript's store anchor walk
(`ompBlobIdentity`, conversation.ts) use it, with the same checks and
order as the Linux code before.
- `fcntl(F_GETPATH)` is not used: it is variadic, and through a fixed
FFI signature on arm64 it returns 0 with an empty path (probed on
eamac).
- Identities come from bigint stats: Bun rounds a Windows file index
held in a number (probe: `…020` vs the exact `…019n`), which would have
refused every picture.
- `OBJECT_ATTRIBUTES`, its `UNICODE_STRING` and the name share the one
allocation passed to `NtCreateFile`, so GC cannot collect a buffer
reachable only through a raw pointer.
- An entry name refuses `/`, NUL, `.`, `..` everywhere, and `\` and `:`
(streams) on Windows only.
- `server/omp-images.test.ts` now runs on all three platforms, the swap
races included, and the native Windows and macOS CI jobs run it.
### Windows: is the held store the one its path names?
Linux and macOS compare the descriptor's own path with the path asked
for. Windows names one directory several ways (a short 8.3 name, as
GitHub's runner keeps its temp directory under `C:\Users\RUNNER~1`;
another letter case), so an exact comparison refused every blob there,
and a case-insensitive one let a different directory through in a
case-sensitive folder. So `isAt` walks the path instead: from its root
(a drive or a share, whose own handle must name that root, so a `subst`
or mapped drive is refused), each name opened through the directory
before it with reparse points refused, down to the very object held,
compared by `FILE_ID_INFO` (the volume serial and the 128-bit id; the
64-bit index where a file system keeps no full id, as FAT32; ids that
name no one file refused). A path with `/`, NUL or the `\\.\` device
namespace is refused before it is walked.
## Try it
On a Mac or Windows PC running this branch's server, in an omp pane (omp
18.8.7 or later):
1. Send omp a prompt with a picture (`@shot.png what is this?`), or ask
it to `read` a picture file.
2. In Chat, the picture shows on the prompt (and on the `read` tool
row). Before this PR it is missing there on macOS and Windows, while
inline pictures still show.
3. Optional check that the store is held: `ls <omp data>/blobs` lists
the picture's sha256 name; the Chat picture is served from that file
only.
## Verification
Fresh clones of origin/main `65ffea69` plus this branch, with
`HERDR_TEST_MODE=unit`, deleted afterwards:
- **macOS** (eamac, macOS 27.0.1 arm64, Bun 1.4.3): `bun test
server/omp-images.test.ts` 21 pass, 0 fail, 3 skip (Windows-only cases);
a real store probe gives `image/png` with the exact bytes. The same
tests against main's server code: 18 fail, probe null. A store under
`$HOME` is served too (`fgetattrlist` names `/Users/...`).
- **Windows** (home-window, Windows 11 26100, Bun 1.4.2, `TEMP` set to
an 8.3 short path as on GitHub's runner): 23 pass, 0 fail, 1 skip (the
colon-name case, POSIX-only by design), including the short-name and
letter-case spellings, a case-sensitive folder (`Box` vs `box`, a
junction whose name differs from its target's only by case), and `/` or
`\\.\` spellings through a junction; probe `image/png`. 20,000 relative
opens with `Bun.gc(true)` every 100: 0 failures. A `subst` drive below a
junction or onto the real folder is refused (probe). Against main's
server code: 18 fail, probe null.
- **Linux**: 21 pass, 3 skip (Windows-only). `bun run check fast` green
on the head (unit 7757 pass, 0 fail).
- The race and swap cases (store or agent directory swapped for a link
around each open, another real store moved in, a link as the store or as
the blob, the data directory swapped before and after it is held) are
refused on every platform.
## Review
- The first head failed CI's Native Windows install: GitHub's runner
keeps its temp directory under an 8.3 short name, and the exact path
comparison refused every blob (reproduced on home-window). Fixed by the
walk above; `isAt`'s design went through three more Codex rounds: case
folding equated distinct directories in a case-sensitive folder
(MEDIUM), `/` or `\\.\` or `subst` could hide a junction above the
walk's root and sentinel file ids compared equal (two MEDIUM), and FAT32
lost its pictures when the full id was required (MEDIUM); each fixed and
verified on home-window. Final round: APPROVE, no findings.
- Codex Sol 6.1 Fast xhigh, round 1: HIGH (Windows name buffers
reachable only through raw pointers; reproduced 28/100,100 corrupt with
mocked symbols under GC), MEDIUM (native CI did not run the file), LOW
(`:` refused on POSIX). All fixed in the second commit; round 2 on that
delta: no findings.
- omo-native-code-reviewer: no code HIGH or MEDIUM. Its HIGH was that
the device results were missing: it read the logs while the runs were
still going; the complete results are above. LOWs: the `fgetattrlist`
argument type was fixed; the macOS firmlink concern was checked on eamac
(served).
- Known limits, all fail closed: Windows refuses every reparse point
(OneDrive placeholders, deduplicated files); a Windows file index with
the top bit set would compare unequal and refuse.
This PR touches a workflow (`.github/workflows/ci.yml`, two test lists)
and a file-serving security path, so it waits for a real review.
`REMOTE_BUNDLE_VERSION` is not raised here: the release PR raises it

Part of #992, the omp and Codex part. It follows #1935 (Claude Code) and #1945 (pi), with the same picture check and the same endpoint rules.
Change
In an omp pane, Chat dropped every picture: a screenshot sent with a prompt, a prompt that was only a picture, and the picture a
readreturned. omp moves a picture's bytes out of the transcript into its blob store and writesblob:sha256:<digest>in their place, so there was nothing to decode. In a Codex pane, a picture a tool returned (view_image, an image generation, a screenshot a command read) was dropped too: only prompt pictures were read. What it does now:
pi-prompt:<entry id>:<place>, and a prompt that is only a picture is a turn of its own and opens one for paging (turnStarts).pi:<call id>:<place>on the tool's row. omp's call ids hold a|(call_…|fc_…), which the ref shape now takes.<data>/sessionsand its blobs at<data>/blobs(getBlobsDir, the samedatabase), soompBlobStorenames the store beside the transcript's real session store (the default agent directory, a profile's,PI_CODING_AGENT_DIR's or the XDG one), by layout alone, whether omp has made it yet or not. A blob is read only by omp's own 64-hex digest and only when its bytes hash to that digest. The image endpoint takes the store's identity from the objects that authorized the pane's transcript (ompAuthorizedTranscript): the ownership check returns the store's real root and the dev:ino of the transcript file it opened, and the agent data directory is then opened once, without following a link and held to the path named, andsessions, the path down to that same transcript file (each component without following a link) andblobsare all reached from that one held descriptor. It reads a blob only from that very directory: the store is opened as a directory without following a link, its descriptor must have that identity (fstatdev:ino) and be the directory the path names (/proc/self/fd), and the blob is opened through that descriptor (O_NOFOLLOW). A session in a--session-dirof its own names no data directory: its blob pictures stay unnamed, and inline ones still show.input_imagedata URLs in acustom_tool_call_outputorfunction_call_outputare namedcodex-tool:<call id>:<place>on the tool's row, for any tool. TheImageViewand image-generation events that repeat them are not read, so each picture shows once. They are servedno-storelike Codex's other refs, so they load only when their row is opened (as feat(chat): show a tool's pictures as small thumbnails on its work line #1952's work line requires).inlineImageType, ompBlobPicture), so the page never names a picture the endpoint refuses.codexHistorySegments, without the bytes a backtrack left). So a lookup never holds the bridge for more than a window's work (Claude's reader and page reads run the same steps straight through, unchanged).HTTP contract:
/api/pane/conversation/imageaccepts one more ref shape (codex-tool:) and answers omp'spi:/pi-prompt:refs. No new endpoint, no new wire field (tool.imagesand image parts already exist).Remote bundle: the server files are in the remote PC bundle.
REMOTE_BUNDLE_VERSIONis not raised here; the release does that (0.4.18 raised it to 37 and its Release run has finished).Design: no new UI: the prompt thumbnails, the work line, the row gallery and the viewer that Claude and pi pictures already use.
Not in this PR: omo and gjc (no recorded sessions with pictures here; they name none, as before), omp's
xd://generate_imagepictures that live only indetails, and local Markdown images in answers.Validation

The one failure is the known slot-admission flake inscripts/check.test.ts(slots > gives its place up when it stops waiting: ticket ports taken on a busy host), in a file this PR does not touch, which passes 66/0 alone. The run before it had only an untouched load-timing failure (scripts/install-node-fallback.test.ts, hung-host kill, 10/0 alone). Counted as passed per the lane's rule for known slot-admission flakes; GitHub CI runs the full suite.
