Fixes #2059 (reported by @Damin-Lee).

Summary

omp 18.10 draws its tool approval ("Allow tool: ") as a panel in the composer's place: the call's details and one key hint (Ctrl+⏎/Ctrl+Q approve · ⎋ deny), with no Approve/Deny rows. parseOmpApproval needed exactly two menu rows, so no reader matched and the pane fell back to the generic card. omp 18.13 adds a menu of choices (Approve, "Approve bash for this session", Deny; grants and a Remember toggle in always-ask-cached) and mounts the composer under the panel.

server/dialogs/omp.ts (parseOmpApprovalPanel) now reads both panels in every symbol preset (words, keycaps, Nerd Font, including macOS's spaced Nerd Font chords):

  • Approve and Deny send the keys the hint names, wherever the cursor is. A Ctrl+letter is preferred (not Ctrl+H/I/J/M), since Ctrl+Enter needs extended key reporting. On 18.13 without one, Approve is answered by moving to its row.
  • 18.13's session and grant rows are answered by moving to them and selecting. The Remember toggle is not offered.
  • A narrow pane that cuts 18.13's hint is read only while the approve keys are still whole (omp takes them before any move), and only while fewer details are shown than omp scrolls at.
  • The terminal answers, with no card: details that scroll, a draft in omp's composer (Enter becomes "deny and send"), a key that cannot be sent (⌘, PgDn), an approve or deny key that is also a move/select key, later output under the panel, and a call omp cut or summarized (…, […Nch elided…], … +N lines in the tool call), since two calls could draw that panel the same.
  • The older Approve/Deny menu reader is unchanged.
  • server/dialogs/live.ts: the omp-approval card stays live while the panel (and 18.13's status row and frame) ends the screen.
  • Approve/Deny are the bridge's own card words, translated (ko/ja/zh), so the 18.10 card is no longer English-only.

Verification

  • Fixtures: 9 screens recorded from real omp 18.10.4 and 18.13.2 in the isolated herdr test session. A fake model provider calls bash ls -la (write for the grants case) under --approval-mode=always-ask/always-ask-cached, at 119 and 57 columns, in each preset. The temporary path is replaced by one of the same length.

  • Tests: server/dialogs/omp.test.ts gains 11 cases. On main, the new reading cases fail. Each review finding has a case that fails on the commit before its fix.

  • Live check: real omp in the isolated test session, an Ovis server on port 0, GET /api/pane/prompt then POST /api/pane/prompt/answer 2.5 s later.

    omp answer tool result
    18.10.4 Approve ran
    18.10.4 Deny denied
    18.13.2 Approve ran
18.13.2 Approve for this session ran
    18.13.2 Deny denied

Every card was kind: approval, Allow tool: bash, with body Command: ls -la. A raw herdr ctrl+enter also approves 18.10.4.

  • bun run check fast passes.

Reviews

  • omo-native-code-reviewer and Codex Sol 6.1 Fast xhigh (adversarial) reviewed the first commit.
  • Codex HIGHs, both fixed:
    • With approve bound to Enter, a cut hint's Deny would have approved.
    • Different calls drawing the same cut panel shared a card id.
  • Codex MEDIUM, fixed: any later output was accepted as omp's composer.
  • omo MEDIUM, fixed: macOS Nerd Font chords such as 󰘴 O were unreadable.
  • omo LOW, fixed: Ctrl+H/I/J/M were accepted as the approve key.
  • The first fix for the card-id HIGH hashed the screen above the panel. The live check caught that omp 18.13's bash box ticks elapsed Ns there, so answers came back prompt_changed. It was replaced by the cut/summarized rule.
  • Codex's re-review of the review fixes passed.
  • Its last round found one more summary form, ast_edit's +N more ops, which now gives no card either.
  • Known limit (written in the code): some omp tools never show their whole call in the approval. edit names only the files; lsp and debug name the action and target. So two such calls draw the same panel. A card read for the first answers the second only in a narrow race: the first was answered in the terminal, and the second was drawn before the browser read the screen again. Every screen-read approval card has this limit. Per the merge policy, a finding that needs such timing is documented rather than blocking.

No REMOTE_BUNDLE_VERSION change: that is the release PR's job.

omp 18.10 draws its tool approval ("Allow tool: <name>") as a panel with
the call and one key hint, and no Approve/Deny rows, so no reader matched
and the pane fell back to the generic card (#2059). 18.13 adds a menu of
choices and mounts the composer under the panel.

- Read both panels in every symbol preset (words, keycaps, Nerd Font).
Approve and Deny send the keys the hint names (a plain Ctrl+letter
first); 18.13's session and grant rows are reached by moving to them.
- A narrow pane that cuts 18.13's hint is answered by moving to each row,
only while fewer details are shown than omp scrolls at.
- Fail closed to the terminal on details that scroll, a draft in the
composer, a key that cannot be sent, or output under the panel.
- Approve/Deny are the bridge's own card words, translated.
- Fixtures recorded from omp 18.10.4 and 18.13.2 in the isolated test
session with a fake model provider.

Fixes #2059
…ad for

Review fixes for the omp tool approval card (#2059):
- Read a hint cut by a narrow pane only while its approve keys are whole,
and leave a menu whose approve or deny key is a move or select key to
the terminal: omp takes those keys before it moves or selects.
- Hash the screen above the panel into the card's id: omp cuts and
summarizes the call, so two calls can draw the same panel.
- Accept only omp's status row and frame under the panel, nothing else.
- Read keys holding a space (Nerd Font on macOS: Ctrl+O as icon, space, O).
- Approve with a Ctrl+letter a terminal cannot confuse with Enter, Tab or
Backspace; without one, move to Approve on 18.13's menu.
Hashing the screen above the panel into the card's id (the previous
commit) changed the id every second on omp 18.13, whose bash box ticks its
elapsed time while the call waits: a live answer came back prompt_changed.
Instead, read a panel only when it shows the whole call. Details cut at
the panel's edge or elided, and multi-line ones summarized, could be drawn
the same for another call, so the terminal answers those.
…ites

ast_edit's approval names its first rewrite and counts the rest
(`+N more ops`), so two calls could draw the same panel. Show no card for
it, as for any call omp cuts or summarizes, and document the tools whose
approval never shows the whole call as a known limit.

An omp approval card can send a Deny key that some terminals read as Enter, approving a tool call the user meant to deny. This needs a non-default Deny binding. The card also does not show when "Remember across sessions" is checked, so a persistent grant row can have a wider effect than the card suggests. Both should be fixed before merge.

… as Enter

A Deny bound to Ctrl+M or Ctrl+J reaches omp as Enter without extended key
reporting, which takes the highlighted row, Approve by default. As for
Approve, such a binding is answered by moving to the row and selecting it.
Review fixes for #2106:
- omp 18.13's "Remember across sessions" toggle, ticked, makes a grant
outlive the session, and the card did not show it: no card while it is
ticked, so the terminal answers where the box shows.
- A regression test drives the merge pass's Deny-alias reproduction through
POST /api/pane/prompt/answer: with Deny bound to Ctrl+J or Ctrl+M and a
legacy terminal, answering Deny must deny (it approved on c2d6304).
- The redraw test asserts a card before comparing ids.

Merge-pass findings on c2d6304, addressed in 75d1024 and c8520f9:

  • HIGH (Codex, reproduced), Deny bound to Ctrl+J or Ctrl+M approved on a legacy terminal.
    • Fix: on 18.13's menu, an approve or deny key in Ctrl+H/I/J/M (OMP_ENTER_ALIAS_RE) is never sent directly. The row is reached by moving to it and pressing the select key the hint names.
    • Regression test: the merge pass's reproduction is ported to server/prompt.test.ts ("denies when omp's Deny is bound to %s…"). It goes through POST /api/pane/prompt/answer with the fake herdr, modelling omp 18.13 on a legacy terminal: Ctrl+J and Ctrl+M arrive as Enter, and Enter takes the highlighted row.
    • On c2d6304: both cases fail (Expected: "denied", Received: "approved").
    • Now: Deny denies, Approve approves, and the session row approves for the session.
  • MEDIUM (omo, reproduced), the ticked "Remember across sessions" toggle was invisible. No card is shown while it is ticked, so the terminal answers. Test: the grants case ticks ☑ and its Nerd Font icon, and expects no card.
  • M2: the redraw test now asserts a string id before comparing ids.
  • CodeRabbit: both threads are answered and fixed. The Deny alias is the same as the HIGH, and the toggle is the same as the MEDIUM.

Local bun run check fast on c8520f9: typecheck, build and generated files pass, and 7731 unit tests pass. The one failure is a 5 s timeout (5026 ms) in scripts/ci-browser.test.ts, which this PR does not touch, on a different test in each of two runs. The PC's load average was about 85 with two reviewers running. Run alone, bun test scripts/ci-browser.test.ts passes 7/7 on this branch and on origin/main. I pushed under the load-flake rule, and GitHub CI runs the full suite.

Re-review of c2d63048..c8520f91 (the merge-pass fixes):

  • Codex Sol 6.1 Fast xhigh: PASS, no blocking findings.
    • Ctrl+J/M Deny now moves to Deny and selects it; Ctrl+H/I get the same guard.
    • The remembered-toggle guard covers the Unicode, Nerd Font and ASCII ticked states.
    • The redraw test no longer passes vacuously.
    • Its sandbox refused the fake herdr socket, so it couldn't run the two endpoint cases. I ran them locally: they fail on c2d6304 (Deny gives approved) and pass on c8520f9.
  • omo-native-code-reviewer: PASS, APPROVE, no CRITICAL/HIGH/MEDIUM.
    • It checked omp's own key matcher, from the 18.10.4 and 18.13.2 installs, with extended key reporting off. LF and CR read only as enter, so the regression test's model of a legacy terminal is accurate.
    • Two LOWs, documented limits where the answer does nothing (the card stays):
      • On 18.10's panel, an approve or deny key bound to Ctrl+H/I/J/M arrives as a key the panel ignores.
      • On 18.13, a select key rebound to Ctrl+J or Ctrl+M makes the rows the bridge moves to do nothing.

…rs it as drawn

The merge pass reproduced, on c8520f9, a Deny that still approved on a
terminal without extended key reporting: Ctrl+Enter and Shift+Enter also
arrive as Enter on omp 18.13 (taking the highlighted Approve or grant row),
and on 18.10 a Deny on Ctrl+J/M approved when Enter was an approve key.

Send a key directly only when it is Esc or a Ctrl+letter other than H, I, J
or M. On 18.13 any other Approve or Deny binding is answered by moving to
its row, with a select key that also arrives as drawn; 18.10 has no rows,
so it gets no card without such a key for both answers. The endpoint
regression test now models a legacy terminal for every alias, keeps omp's
default Ctrl+Enter approve binding, and wraps the cursor as omp does.

Merge-pass blocker on c8520f9 (Deny that arrives as Enter on a legacy terminal), fixed in 1c42d56.

Rule: the bridge now sends omp a key only when every terminal delivers it as drawn: Esc, or a Ctrl+letter other than H, I, J or M (OMP_DIRECT_KEY_RE).

  • omp 18.13: any other Approve or Deny binding is answered by moving to its row and pressing the select key. The select key must itself arrive as drawn (Enter, Space or a direct key), or there is no card. This covers Ctrl+⏎, ⇧⏎, Ctrl+J/M and Ctrl+H/I.
  • omp 18.10: the panel has no rows to move to, so there is no card unless both Approve and Deny have a direct key. This closes ⏎ approve · Ctrl+J deny and also the review's L1, a Ctrl+⏎-only Approve that answered nothing.
  • Collision checks: sent keys now always arrive unchanged, so comparing raw binding names (approve or deny vs ↑/↓/select, approve vs deny) compares what omp receives.
  • The default setup still gives a card: Ctrl+⏎/Ctrl+Q approve, ⎋ deny, ⏎ select. Approve sends Ctrl+Q and Deny sends Esc.

Evidence:

  • Merge pass's reproduction: c8520f91/omp-adversarial.test.ts, which drives omp 18.13.2's real ApprovalDialogComponent through POST /api/pane/prompt/answer, passes 8 of 8 against 1c42d56. Deny bound to Esc, Ctrl+J, Ctrl+M, Ctrl+⏎ and ⇧⏎ denies, and so does Deny with a grant row highlighted. Its two Remember-toggle cases pass too.
  • server/prompt.test.ts regression test: it now models a legacy terminal for every alias and keeps omp's default Ctrl+⏎ approve binding, so it no longer mirrors the fix's own list. On c8520f9 its Ctrl+⏎ and ⇧⏎ cases fail; on 1c42d56 all 6 pass.
  • Parser cases in omp.test.ts:
    • 18.10 gives no card for ⏎ approve · Ctrl+J deny, Ctrl+Q/⏎ approve · Ctrl+M deny, Ctrl+⏎ or ⇧⏎ Deny, and a Ctrl+⏎-only or ⌥⇧A Approve.
    • 18.13 moves to Deny for Ctrl+⏎, ⇧⏎ and Ctrl+H/I/J/M.
    • 18.13 gives no card when the select key is Ctrl+M.
  • bun run check fast: green, with 7736 unit tests, typecheck and build.

Re-review of c8520f91..1c42d562 (the bridge sends only keys every terminal delivers as drawn):

  • Codex Sol 6.1 Fast xhigh: PASS, no blocking findings.
    • The restricted keys close the reported legacy-terminal alias class.
    • The default setup (Ctrl+⏎/Ctrl+Q approve, ⎋ deny, ⏎ select) keeps its card on both versions, in every shipped symbol preset.
    • Its sandbox refused the fake-herdr socket again. The endpoint cases and the merge pass's real-omp reproduction (8/8) were run locally, as above.
  • omo-native-code-reviewer: PASS, APPROVE, no blockers.
    • Every key sent arrives unchanged.
    • omp checks approve first, and it cannot share a binding with deny.
    • The moves and the select key are restricted the same way.
    • 18.10 needs a direct key for both answers.
    • omp.test.ts passes 21/21, and the six endpoint cases pass 6/6.

Known limits, all nonblocking (custom bindings only, and none approves on Deny in the default setup):

  • Deny on omp's details key (Codex P3): with Deny on Ctrl+O, omp's default details key, omp opens the details first, and the approval stays pending.
  • Approve bound to Ctrl+M (omo LOW): with Approve on Ctrl+M as well as ⏎ select, a moved-to row could approve on a legacy terminal, if omp reads CR as Ctrl+M (not verified).
  • omp 18.10 with only Ctrl+⏎ or an Alt chord to approve (omo LOW, intended): no card, so the terminal answers.
  • Ctrl+H in the endpoint test (omo LOW): covered only by the parser case in omp.test.ts.

Merging 1c42d56 after three merge-pass review rounds. The Deny-approves cases are fixed: Ctrl+J/M, then Ctrl+Enter/Shift+Enter and 18.10's collision. omo's binding matrix shows 0 wrong answers in 540 cards, against 259 on c8520f9. Remember-ticked grants give no parsed card. Both reviewers pass this head, and the required checks are green. CodeRabbit did not review this head. Codex's fallback-card MEDIUM and omo's LOWs go to a follow-up issue.

…) (#2245)

Follow-up to #2106 (#2059). Found while preparing the 0.4.20 test guide:
on live omp 18.13.2 the tool approval ("Allow tool: bash") still showed
the generic "Waiting for input" card.

## What it does for the user

omp's tool approval card now shows whatever composer omp draws under the
panel, not only the default status band: `composer.shape: box` (the
config that hit it), and the Claude Code, Pi, borderless, rule, field
and rail shapes, with any status-line preset and the ascii symbols.
omp's Ask card now also shows under the rule, field and rail shapes.
Answers still go through `POST /api/pane/prompt/answer` with the keys
the panel names.

## Cause

omp 18.13 mounts its composer under the approval panel (pi-tui
`extension-ui-controller.ts` `showCollabAwareApproval`). #2106 read only
the band shape's two rows (`⠹ 0s …` then `╰─`), and the status row had
to start with a spinner and a time. With `composer.shape: box` omp draws
`╭── … ──╮` / `╰─ … ─╯` instead, so the panel was not at the bottom and
no reader matched. The Ask card's status-row check (#1812) also wanted
that row right under the panel's border, but rule, field and rail keep
one empty row over their status bar (`bottomBarGap`).

## How

- `server/dialogs/omp.ts`: `OMP_COMPOSERS` lists the rows each of omp
18.13.2's eight built-in shapes draws when empty
(`pi-tui/src/components/composer/*.ts`), in the unicode and ascii box
glyphs, including the box and field rows of omp's IME-safe cursor
layout. `ompAboveComposer` takes one of them off the bottom of the
screen; the panel must then end right above it. The status row is read
by its place only, since the user's status-line settings choose its text
(the minimal preset has no spinner).
- The status row is read by its place and by how omp starts it (a
segment group's pad space, the band's powerline cap, or with no left
group the band's gauge rule), never by its words. omp's own cursor in
the empty prompt (`showHardwareCursor: false`) and a status line with no
segments (omp then leaves out the row and its gap) read too.
- Anything else under the panel matches no shape, so the terminal
answers: a draft in any shape's prompt (omp then also redraws the hint
and the Deny row), later output, a hook's status line, an extension's
own composer shape, a box border omp does not draw, and a bare
borderless `❯` with no status under it (a shell draws that too).
- `server/dialogs/live.ts`: the Ask card's status row may sit under one
empty row.

## Verification

- **Fixtures:** omp 18.13.2 recorded in the isolated test herdr with a
fake provider, `--approval-mode always-ask`: one approval screen per
shape, the band and box shapes with the minimal status-line preset, box
and claude with the ascii presets, box in a 59-column pane, and the Ask
panel over rule, field and rail. Plus the live boxed screen from the
test guide (a user's own config). Run root paths are replaced by ones of
the same length.
- **Tests:** `server/dialogs/omp.test.ts` gains 8 cases. On main's
`omp.ts`/`live.ts`, 3 fail (every shape, the live box screen, the Ask
over a gap). The fail-closed cases (a draft in each shape's prompt,
output after the composer, another composer shape or box border, a panel
left over lines shaped like a composer, output in the Ask's gap) pass on
both. Each review fix was mutation-checked: dropping the IME-safe rows,
the software cursor, the status-row start or the box border's corners,
or letting `❯` take a draft, fails a case.
- The reviewers' rendering probes (omp's own `Editor` and composer
styles, both symbol presets: software cursor, IME-safe layout, no status
segments, right group only, hook status) give the intended verdict on
all of them.
- **Live check** (on the final commit, and before the review fixes):
real omp 18.13.2 in the isolated test herdr, an Ovis server on port 0,
`GET /api/pane/prompt`, then `POST /api/pane/prompt/answer` (200 each
time); the fake provider recorded omp's tool result.

  | shape | answer | card | tool result |
  |---|---|---|---|
  | box | Approve | approval, Allow tool: bash | ran |
  | box | Approve bash for this session | same | ran |
  | box | Deny | same | denied |
  | claude | Approve | same | ran |
  | rail | Deny | same | denied |
  | pi | Deny | same | denied |

- Every recorded capture (8 shapes × default, minimal and ascii presets,
and a 59-column pane) gives the approval card.
- `bun run check fast` passes on the final commit (typecheck, build,
8145 unit tests).
- The omp, prompt, submit, pending-input, conversation-push and api
contract files: 388 pass. The 4 failures are api.contract conversation
tests whose fixture Grok, Agy, Muse and Codex processes never start on
this machine, and they fail the same way on 9f85caf, before this
change.

## Only while omp holds the input (merge-pass review)

The merge pass reproduced the HIGH declined below with a real omp: herdr
0.9.5 keeps naming a pane omp while its omp is **stopped behind the
shell** (job control). The screen still showed omp's approval panel
under the shell's lines, the card was offered, and `POST
/api/pane/prompt/answer` sent Ctrl+Q to bash.

- `server/prompt.ts` `ompOwnsInput`: an omp pane gets a card (any omp
reader's, or the fallback card) only while one of omp's own processes is
in the pane's foreground (`pane.process_info`, `agentProcesses(…,
"omp")`, which picks out omp itself, whether its own binary or bun
running its script), not herdr's retained agent label. Live-verified: a
running omp is in front; stopped, the foreground is `/bin/bash` alone
while herdr still says omp.
- The answer checks it again right before each key it sends
(`sendStep`); otherwise it stops with 409 `prompt_changed`. herdr not
answering counts as not omp.
- Windows is read as before: it has no job control and no foreground
process group (`process_info` names only the shell there).
- This covers #2106's 18.10 panel and every other omp reader (Ask,
review, model picker, /handoff), which shared the same guarantee.

Verification:
- `server/omp.contract.test.ts`, event-driven on real herdr: a stand-in
omp draws 18.13's panel and 18.10's, is stopped behind its shell, and
the shell draws the dialog it left and reads one raw byte. No card, a
409, and the first byte the shell reads is the test's own. On 9f85caf
both panels give the card, and the answer's key reaches the shell.
- `server/prompt.test.ts`: omp stopped after the first key of a custom
answer (whose text and Enter go unseen) sends nothing more and answers
409. Removing the check before each key fails it. The fake herdrs now
report omp in front for a live omp.
- Live, with real omp 18.13.2, an isolated herdr and Ovis on port 0:
running, Approve ran and Deny was denied. Stopped behind bash over the
stale box panel: `GET /api/pane/prompt` returned no card, the POST
returned 409, and bash's first byte was the test's.
- Codex Sol 6.1 Fast xhigh reviewed the fix:
  - MEDIUM, fixed: the unit fakes had no omp in front.
- HIGH, residual, documented: a stop between the last check and herdr
writing the key is not seen. herdr's `pane.send_keys` takes no condition
on the foreground, so the check runs immediately before each key. A
guarded send in herdr would close this.
- HIGH, declined: a foreground group holding omp *and* another process
passes. herdr lists every process in the foreground group, and omp's own
children (LSP servers, tools) share omp's group. Rejecting mixed groups
would drop live cards, and herdr cannot say which process reads the
terminal. A shell in front of a stopped omp is in its own group, which
is the case reproduced.

## Reviews

- omo-native-code-reviewer reviewed the first commit: approve, no
blockers.
- MEDIUM, fixed: the IME-safe box and field rows (from omp's source, not
recorded) had no test.
- LOW, fixed: a draft was tested in three shapes only; now in all eight.
- LOWs noted, unchanged: the status row is read by place; a hook's
status line or an empty claude/rule left group gives no card (as before
for band); the Ask's one-row gap is accepted in every shape (the panel's
hint row still anchors it).
- Codex Sol 6.1 Fast xhigh (adversarial) reviewed the first commit:
- HIGH, fixed: any line in the status row's place kept a panel left over
another program's lines live. The status row must now start as omp draws
it, and the box's top border needs a rule out of each corner.
  - MEDIUM, fixed: omp's software cursor broke every shape.
- MEDIUM, fixed: a status line with no segments (no row, no gap) gave no
card.
- Codex's re-review of the fixes:
- MEDIUM, fixed: the ascii band with only a right group (`-…< [M] …`) no
longer read.
- HIGH, declined at first, then reproduced by the merge pass through a
stopped omp. Fixed by the ownership gate above.

## Known limits (not changed here)

- omp's Ask card under a status bar without omp's spinner (the minimal,
compact and ascii status-line presets) with a shape that puts the bar
under the panel (pi, claude, borderless, rule, field, rail) still shows
the generic card: under the Ask panel, that row is the only evidence it
is not later output, and its text is the user's choice. Fails closed.
- omp's Ask panel in the ascii symbol preset is not read in any shape
(the Ask reader looks for `╭`); it was not before either.
- A hook or extension status line under omp's status bar shows no
approval card, as for the band shape before. So does a bare borderless
composer with no status segments.

OMP tool approval shows a broken fallback card: omp's "Allow tool" dialog has no Approve/Deny rows
