Queue workers can currently remain stuck when CPU-bound PHP prevents the coroutine timeout monitor from running. A separate problem occurs when a job releases itself and then throws: failure handling can publish another outcome for an attempt that has already handed its reservation back to the queue.

This PR makes timeout handling work when the event loop is blocked, prevents failure handling from racing a still-running attempt, and preserves completed job handoffs.

Worker timeouts

For the default mode, which stops the worker after a timeout:

  • Stop accepting jobs when the timeout is declared, notify the job, and cancel its coroutine. Wait for expired coroutines together for the monitor interval plus one second, normally two seconds.
  • Apply timeout failure settings after the coroutine has actually finished. If cleanup is still running after that wait, terminate without running failure handling concurrently with it. Any remaining reservation becomes available again through the queue's normal expiry rules.
  • Preserve an attempt that completes, releases, or fails while unwinding cancellation. A timeout does not replace an outcome the job has already published.
  • Keep the timeout monitor active when a normal shutdown is already waiting for jobs. Timeout termination takes precedence over a normal exit caused by an admission or time limit.

When PHP blocks the event loop, an in-process PCNTL alarm suspends the blocking coroutine. This lets the existing monitor apply timeout policies and run failure callbacks before stopping the worker. A second alarm uses the kernel's default termination action if this handling blocks again or exceeds its deadline. There is no additional process or service.

Ordinary cancellation and blocked-loop handling have different cleanup limits. When the event loop stays responsive, yielding failure callbacks may finish normally after the job has unwound. Once the alarm has suspended a blocking coroutine, all remaining handling must finish within the additional alarm deadline, normally two seconds. Native calls that block signal handling cannot be interrupted by this mechanism.

The alarm is owned by the daemon and is not installed for --once or direct job processing. An independent timer tracks event-loop progress so a slow, yielding failure callback does not look like a blocked loop. Job admission updates the alarm deadline without scanning the running jobs; scans occur when the alarm fires.

With Worker::$killOnTimeout = false, the job coroutine handles the timeout after its code unwinds, including retry, backoff, and FailOnTimeout. Catching cancellation without completing, releasing, or failing the job still applies those settings. If the timeout notification is publishing an outcome, the body waits for it before deciding what remains to do. Once job execution ends, the worker's subsequent failure handling and completion listeners finish without another timeout cancellation. Other jobs continue running.

Released attempts and failure handling

A worker no longer fails or releases an attempt again when its handler throws after releasing it. The original error is still reported, and the next delivery enforces the job's attempt limit. FailOnException and ThrottlesExceptions also preserve inner release, deletion, and failure outcomes.

Errors from timeout notification, failure callbacks, marker cleanup, or timeout listeners are reported without abandoning other expired attempts. Processing-marker cleanup remains serialized where a notification can release a job while its body finishes in another coroutine.

If the exception reporter itself fails, the worker falls back to PHP's error log so timeout policies, job fetching and lost-connection shutdown still run. Cancellation during reporting continues to propagate.

Normal worker termination runs stopping listeners and the custom kill callback. If either throws, its error is reported and termination continues, even if the exception reporter also throws. During blocked-loop recovery, emergency termination can occur before these callbacks finish.

Cancellation through cleanup

A finally block can throw while unwinding CanceledException, leaving cancellation in the previous-exception chain. Direct-only checks treated these errors as ordinary job failures, which could trigger another retry or publish completion events for canceled work.

Coroutine::causedByCancellation() recognizes these chains while stopping at ForeignCancellationException. ChildCancellationException extends that boundary: an independently canceled child remains an ordinary failure for its active caller. Shared container resolutions, image sources and gRPC connections also preserve this distinction when passing an owner's failure to other callers.

The worker, queue middleware, synchronous execution, job failure hooks, and event dispatcher use that distinction. Waiter, Scout imports, AI broadcast senders, and Saloon pools preserve the child boundary when cleanup replaces cancellation. Cleanup errors remain available for reporting, and ordinary child failures retain their normal retry or failure behavior.

Connection release listeners and Saloon token acquisition preserve the canceled owner's exception through cleanup. Waiting token callers may retry instead of inheriting that cancellation.

Direct worker calls propagate caller cancellation; daemon jobs report cleanup errors once according to the worker's reporting setting. Console commands and scheduled tasks preserve cancellation through output, mutex cleanup and deferred callbacks, instead of replacing it with an earlier ordinary error or starting completion work after cancellation.

Retry and rescue helpers, HTTP requests, cache and queue failover, credential refresh, and paginated requests also stop when cleanup carries cancellation. Cache rollback still completes the cleanup it owns. AI tools do not turn cancellation into a result or execute the next approved tool; conversation persistence saves a completed turn without starting more title work.

gRPC caller cancellation does not cache a failed response, consume a buffered response, finish observers, or advance retry backoff. Server deadlines still return DeadlineExceeded when service cleanup throws, while reporting the cleanup error for diagnosis.

The coroutine API uses parentId() without the redundant pid() alias, and reportExceptions(bool $report = true) controls automatic reporting. Contributor guidance now covers cleanup exception chains and cancellation ownership; a tracked investigation will assess whether PHPStan can catch these mistakes without flagging valid native-only checks.

Validation

Real worker subprocess tests use signals and SQLite to verify failure records, reservation expiry and retry, yielding failure callbacks, unfinished cleanup, release during cancellation, suspension during timeout notification, and termination when handling blocks again.

Additional regressions cover durable release-then-throw behavior, exception accounting, worker shutdown, processing-marker ownership, completion listeners, and cancellation across the affected packages. Formatting, source and type analysis, and the affected queue, coroutine, container, connection-pool, image, gRPC, events, scheduling, Scout, Saloon, and AI broadcast checks pass. Local integration checks include SQLite and an isolated Redis instance.

Note

Handle blocked queue workers and detect cancellation through exception chains

  • Adds Coroutine::causedByCancellation, which walks an exception's previous-exception chain to detect cancellation, and stops at ForeignCancellationException to preserve coroutine boundaries. Replaces direct CanceledException catches with this check across the framework (console, cache, events, gRPC, HTTP, queue, AI tools, coroutine, connection pool).
  • Rewrites Worker.php timeout handling. The worker now arms a PCNTL alarm for job deadlines, distinguishes kill-on-timeout and non-kill modes, waits for interrupted coroutines during a grace period, and terminates the process when the event loop is blocked past a deadline and grace period.
  • Preserves job outcomes: timeout failure policy is skipped for jobs already failed, deleted, or released; released attempts no longer get a second failed outcome; non-kill timeouts coordinate notification with job completion via a WaitGroup.
  • gRPC changes: cancellation from foreign coroutines is wrapped in ForeignCancellationException in Connection.php; HandleCall and ResponseFactory gain an ExceptionMapper dependency and report non-native cancellations through it.
  • Risk: behavioral changes — Coroutine::enableReportException is replaced by Coroutine::reportExceptions/flushState, Coroutine::pid is removed, and ChildCancellationException now extends ForeignCancellationException. Wrapped cancellation exceptions now propagate as cancellation instead of being treated as ordinary failures at many handlers.

Summary by CodeRabbit

  • New Features
    • Queue workers now recover from blocked event loops during timeouts and apply timeout handling more reliably.
    • Coroutine cancellation detection now recognizes cancellations wrapped by cleanup errors across framework operations.
  • Bug Fixes
    • Queue jobs that have already been released, deleted, or failed are no longer handled as failed a second time.
    • Cancellation and cleanup failures are propagated more consistently without triggering unrelated retries or failure callbacks.
  • Documentation
    • Updated guidance explains queue timeouts, cancellation cleanup, and gRPC deadline behavior.
  • API Changes
    • Exception reporting is now controlled with Coroutine::reportExceptions(); the Coroutine::pid() alias was removed.

Cleanup can replace a cancellation exception while retaining it in the previous-exception chain. Recognize that cancellation without treating an independently canceled child as cancellation of its active parent.

Add Coroutine::isCancellation and stop classification at ChildCancellationException. Waiter uses the same rule when returning child failures, preserving the cleanup error as the previous exception. Document the distinction for low-level error handling.

Extend the cancellation tests with cleanup errors and nested child boundaries. Formatting, static analysis and the coroutine suite pass.
An import child can throw from cleanup while unwinding cancellation. Forwarding that error unchanged makes its cancellation chain look like cancellation of the caller.

Wrap this case in the existing ChildCancellationException while retaining the cleanup error and first-failure behavior. Ordinary import failures remain unchanged, and active imports still finish before the caller receives the failure.

Extend the independent-cancellation regression to cover yielding cleanup that throws. The Scout suite and static analysis pass.
A broadcast sender whose cleanup throws during cancellation must remain a failed child operation. Its active producer must not inherit cancellation merely by inspecting the exception chain.

Classify the sender error before handing it back and retain the cleanup throwable inside ChildCancellationException. Channel closure and producer disposal continue through the existing paths.

Cover direct cancellation and cleanup errors through real broadcast listeners. Broadcast batching tests, SQLite queue integration and static analysis pass.
PoolException exposes a child failure through its previous exception. When request or callback cleanup replaces cancellation, forwarding the raw error lets callers mistake a child failure for cancellation of the whole operation.

Recognize cancellation chains at each existing request and callback boundary. Preserve successful responses, original request failures and callback failures, and continue bypassing the exception handler for canceled requests.

Extend the pool regressions across request, response-handler and exception-handler cancellation, including throwing cleanup. The Saloon suite and static analysis pass.
Cancellation must stop event observation even when a listener or observer throws another error from cleanup. Direct-only cancellation catches could run later observers or replace that cancellation with an earlier ordinary failure.

Use the shared cancellation classifier at the existing dispatch boundaries. Preserve the original cleanup error, keep independently canceled children as ordinary failures, and retain first-error handling for ordinary observer errors. Declare the direct coroutine dependency.

Extend the dispatch regressions for wrapped cancellation and independent child failures. The events suite and static analysis pass.
A cleanup error chained to cancellation must not restart job failure handling or publish completion events. The base job and synchronous queue previously recognized only direct cancellation exceptions.

Preserve cancellation through batch rollback, failed hooks, job execution and failure observation. Keep ordinary failure publication unchanged and rethrow cleanup errors for the caller to report. The shared synchronous execution path also covers coroutine-backed queues.

Extend the existing cancellation tests to cover yielding cleanup errors and failure listeners. Queue tests and static analysis pass.
Exception middleware must not fail an attempt after it has released its reservation, or replace an inner failure or deletion with another release. It must also let cancellation escape when cleanup has replaced the original cancellation exception.

Guard FailOnException after its predicate so a predicate that releases the job is respected. ThrottlesExceptions preserves existing terminal outcomes and bypasses accounting and callbacks for cancellation of the current operation. Independently canceled children still follow normal failure policies.

Cover release from the body or predicate, nested terminal actions, wrapped cancellation and normal policy handling of child failures. Queue integration tests and static analysis pass.
CPU-bound PHP can prevent the coroutine timeout monitor from running. Add a daemon-owned PCNTL alarm that suspends the blocking coroutine so the monitor can handle the timeout. A kernel alarm terminates the process if that handling stalls. Track event-loop progress separately so normal yielding failure callbacks retain time to finish.

Stop admissions when kill-mode timeouts are declared. Notify and cancel expired jobs, then wait for their coroutines to finish before applying failure policies. Leave unfinished attempts reserved instead of racing their cleanup. Handle already suspended attempts first, and never cancel or reassign an attempt suspended after timeout declaration.

In non-kill mode, apply timeout policies after the job unwinds and stop monitoring when execution finishes, allowing subsequent failure handling and completion listeners to finish normally. Report cleanup errors once, isolate timeout callback errors, and keep timeout termination ahead of normal worker shutdown.

Preserve released attempts when their handlers later throw. The next delivery enforces attempt limits without publishing a second outcome for the old reservation. Retain processing-marker serialization where notification and body cleanup overlap.

Add real subprocess signal tests backed by SQLite, plus worker, middleware-interaction and durable handoff regressions. Document cancellation, callback timing and retry guarantees. Formatting, source and type analysis, and the affected queue, coroutine, event, scheduling and consumer suites pass.

Walkthrough

The change adds Coroutine::causedByCancellation() and applies it across framework components to recognize cancellation in exception chains while respecting foreign-cancellation boundaries. Queue workers also add bounded timeout unwinding and PCNTL-assisted recovery when the event loop is blocked.

Changes: Coroutine cancellation and framework handling

Layer / File(s) Summary
Cancellation classification and ownership boundaries
src/coroutine/*, src/docs/coroutines.md, tests/Coroutine/*
Coroutine::causedByCancellation() examines previous exceptions and stops at ForeignCancellationException. ChildCancellationException extends that boundary type. Coroutine reporting APIs and state reset behavior are updated.
Cancellation handling across framework components
src/ai/*, src/cache/*, src/connection-pool/*, src/container/*, src/database/*, src/events/*, src/foundation/*, src/grpc/*, src/http/*, src/image/*, src/queue/*, src/saloon/*, src/scout/*, src/support/*, src/console/*
These components use the shared classifier in place of direct CanceledException checks. Tests cover wrapped cancellation, cleanup failures, and child-cancellation behavior.
Queue timeout handling and outcomes
src/queue/src/Worker.php, tests/Queue/*, src/docs/queues.md, src/docs/porting-from-laravel.md, src/queue/README.md
The worker interrupts expired jobs, waits through a bounded unwind period, and uses PCNTL alarms for blocked-loop recovery. Tests and documentation describe timeout outcomes, retry state, and termination behavior.

High-Level Assessment

The following are alternative approaches to this PR:

1. External worker watchdog
  • ➕ Could terminate workers when PHP cannot dispatch signals.
  • ➖ Requires another process or service.
  • ➖ Cannot run the worker's in-process failure callbacks before termination.

Recommendation: Use the in-daemon alarm for this change: it restores the existing monitor and failure lifecycle without new infrastructure. An external watchdog could separately cover native calls that prevent signal dispatch, but would not replace safe in-process outcome handling.

Action required

1. Completed jobs are reported as timed out ✓ Resolved
Description
terminateTimeoutJobs() marks every expired job as timed out before notifying any of them, then
processes the snapshot without checking whether each job is still running. If the first job's
notification yields and a later job finishes, that job's normal completion event is suppressed and
the worker emits a timeout event even though its queue outcome was already published.
Code

src/queue/src/Worker.php[717]

+                $this->timeoutJobIds[$jobId] = $job['expires_at'];
Evidence
The scan sets timeout markers for all expired jobs before invoking notifications, which can run user
callbacks. A completed job still has its marker when process() checks it, and the final snapshot
loop emits a timeout event without checking whether the job remains in runningJobs.

## Issue description
The kill-mode timeout scan marks all expired jobs before notifications, which can yield. A job that completes during an earlier notification retains its timeout marker and receives a false timeout event instead of its normal completion event.
## Fix Focus Areas
- src/queue/src/Worker.php[708-760]
- src/queue/src/Worker.php[1205-1211]
## Recommended Fix
Before declaring each job timed out, verify that its attempt is still running. Avoid setting its timeout marker in the initial snapshot so a job that finishes while an earlier notification yields can publish its normal outcome.

2. Unrelated jobs are treated as canceled after a shared-resolve failure ✓ Resolved
Description
Coroutine::isCancellation() returns true for any exception that has a CanceledException anywhere
in its previous chain, unless a ChildCancellationException comes first. SharedResolution::fail()
and ImageSource deliberately wrap the owning coroutine's cancellation in an ordinary
BindingResolutionException/ImageException for other waiters. The worker, SyncQueue,
Job::fail, FailOnException, ThrottlesExceptions and the event dispatcher now treat those
waiters' failures as their own cancellation, so they skip failure handling, release, marker cleanup
and JobAttempted, and leave the job reserved with no recorded outcome.
Code

src/coroutine/src/Coroutine.php[R261-269]

+        do {
+            if ($exception instanceof ChildCancellationException) {
+                return false;
+            }
+
+            if ($exception instanceof CanceledException) {
+                return true;
+            }
+        } while (($exception = $exception->getPrevious()) !== null);
Evidence
SharedResolution's comment states that waiters must see an ordinary failure "so their own requests
and jobs do not stop as if canceled", but it keeps the CanceledException as previous. The new
helper walks getPrevious() and returns true when it reaches that exception. In Worker::process, an
exception classified this way sets $canceled = true and is rethrown, which skips
handleJobException and the finally block's marker cleanup and JobAttempted dispatch. ImageSource
caches an ImageException built the same way and serves it to later readers.

## Issue description
`Coroutine::isCancellation()` treats any exception with a `CanceledException` in its previous chain as cancellation of the current operation. `SharedResolution::fail()` and `ImageSource` intentionally convert another coroutine's cancellation into an ordinary failure (`BindingResolutionException`, `ImageException`) but keep the cancellation as `previous`. Queue and event callers now treat those waiters' failures as cancellation and skip failure handling.
## Fix Focus Areas
- src/coroutine/src/Coroutine.php[259-272]
- src/container/src/SharedResolution.php[46-55]
- src/image/src/ImageSource.php[66-75]
## Recommended Fix
Pick one of these:
- Introduce a marker interface or base class (for example, `ForeignCancellationBoundary`, or reuse `ChildCancellationException`) and make `isCancellation()` stop and return false when it reaches it. Make SharedResolution's and ImageSource's wrappers implement or extend it.
- Or stop passing the foreign `CanceledException` as `previous` in those wrappers, and put its message in the wrapper's text instead.
Add regression tests showing that `isCancellation()` returns false for these wrappers and that a worker job receiving one is released or failed normally.

Description
startTimeoutAlarm() replaces the process-wide SIGALRM handler without saving it, and daemon
cleanup sets the handler to SIG_DFL. If an application had installed an alarm handler before
running the daemon, that handler is lost after the worker exits normally.
Code

src/queue/src/Worker.php[R481-482]

+                    pcntl_alarm(0);
+                    pcntl_signal(SIGALRM, SIG_DFL);
Evidence
Alarm installation does not retain the previous process-wide handler, while normal cleanup
explicitly installs the default rather than restoring it.

## Issue description
The daemon replaces the process-wide alarm handler and resets it to the default on normal exit, discarding a handler installed by the application.
## Fix Focus Areas
- src/queue/src/Worker.php[619-631]
- src/queue/src/Worker.php[480-484]
## Recommended Fix
Save the existing SIGALRM disposition before installing the worker handler and restore it during normal daemon cleanup, alongside cancellation of the worker-owned alarm.

4. Worker can hang forever, ignoring SIGTERM, if the timeout monitor stops 🐞 Bug ☼ Reliability
Description
In kill mode, the new branch at the top of the daemon loop (`static::$killOnTimeout &&
$this->hasTimeoutJobs()`) only sleeps and continues. It no longer drains signals or runs the stop
checks, so only the monitor's later kill() can end the process. If a cancellation escapes
terminateTimeoutJobs() (reportTimeoutException() rethrows cancellations raised by timeout
notification, failure callbacks or JobTimedOut listeners), Timer::tick turns a direct
CanceledException into STOP; the monitor never reaches kill(), timeoutJobIds stays
populated, and the worker spins indefinitely without acting on SIGTERM/SIGINT.
Code

src/queue/src/Worker.php[R335-339]

+                if ($this->recoveringBlockedLoop || (static::$killOnTimeout && $this->hasTimeoutJobs())) {
+                    $this->sleep(self::SHUTDOWN_WAIT_SECONDS);
+
+                    continue;
+                }
Evidence
reportTimeoutException rethrows cancellations. The monitor tick calls kill() only after
terminateTimeoutJobs returns. Timer::tick catches a direct CanceledException from the callback,
returns STOP and breaks out of its loop, so the monitor stops for good. Timeout IDs are never
cleared in kill mode, and the daemon's new early branch skips drainPendingSignals and
stopIfNecessary, so nothing else can end the loop.

## Issue description
In kill mode, once a timeout is declared, the daemon loop only sleeps and waits for the monitor to call `kill()`. If timeout handling throws a cancellation, the Timer stops the monitor and `kill()` never runs. The worker then loops forever and ignores interruption signals.
## Fix Focus Areas
- src/queue/src/Worker.php[590-605]
- src/queue/src/Worker.php[335-339]
## Recommended Fix
In the monitor callback, wrap `terminateTimeoutJobs($options)` in try/finally and run the `if (static::$killOnTimeout && $this->hasTimeoutJobs()) { ... kill(...) }` block from the `finally`, so termination happens even when handling throws. As a backstop, have the daemon's sleep branch still drain pending signals, or call `kill()`/`terminateProcess()` if the monitor timer is no longer running.

This scenario requires a callback to cancel the timeout monitor itself or throw a synthetic cancellation. The worker cancels job coroutines, not the monitor, and no supported path was found that produces the proposed monitor cancellation. Ordinary callback/reporting failures are logged by Timer and the next tick continues. The separate verified shutdown-hook problem is fixed in Worker::kill(), which now guarantees termination even when a stopping listener, kill callback or exception reporter throws. No additional monitor watchdog is needed.

Introduce ForeignCancellationException as the boundary for a cancellation received from another coroutine. ChildCancellationException extends it, and cancellation classification stops at the first matching boundary or native cancellation in the exception chain.

Parallel now recognizes cleanup errors chained to cancellation before publishing a child failure. Preserve the original exception so active callers can report the error without treating themselves as canceled.

Extend existing cancellation and parallel coverage for native and chained failures, and document the classification order. Validated with the coroutine suite, formatting and static analysis.
A canceled shared-service resolver must not cancel its waiters through a previous-exception chain. Wrap the published failure in the foreign cancellation boundary while preserving BindingResolutionException and the original cause.

Recognize cleanup exceptions chained to cancellation as well as bare cancellation. Declare the coroutine dependency used directly by the container, and extend the existing concurrent resolution regression to cover throwing cleanup and a subsequent successful resolution.

Validated with the container suite, formatting and static analysis.
Image source readers share a one-time resolver and its result. When its owner is canceled, preserve the owner's original exception but publish an ordinary ImageException with an explicit foreign cancellation boundary to other readers.

Handle cleanup errors chained to cancellation without retrying a potentially partial read. Extend the concurrent source regression to verify both the owner and waiter exception chains for bare cancellation and throwing cleanup.

Validated with the image suite, formatting and static analysis.
Connection release listeners can throw during cancellation cleanup. Classify the full exception chain so these failures propagate to the canceled owner instead of being logged and swallowed as ordinary listener errors.

Keep resource release and database lease cleanup unconditional, and preserve the primary cancellation when release also fails. Retain reporting of secondary cleanup errors rather than suppressing them merely because they contain cancellation.

Extend existing release and failure-precedence cases for chained cancellation. Validated with the connection-pool suite, SQLite pooled connection integration tests, formatting and static analysis.
Treat a cleanup exception chained to cancellation as cancellation of the token acquisition owner. Leave acquisition retryable for waiting callers instead of caching that owner's failure as their result.

Apply the same classification when a pool producer or idle-connection release is canceled, so admitted requests are canceled and the owner receives its original failure. Extend the existing real-cancellation cases to cover throwing cleanup and bounded child cleanup.

Validated with the Saloon suite, formatting and static analysis.
A connection owner's cancellation is an ordinary connection failure for other active streams. Publish that failure with an explicit foreign cancellation boundary while retaining its original cause.

Recognize cancellation chained through callable deserializer cleanup, initial attempt setup, retries and operation observers. Do not cache a canceled caller's failure, consume its buffered response, finish its observers or retain advanced retry backoff. A subsequent caller can continue the operation normally.

Declare the direct coroutine dependency and extend existing regressions for shared-stream isolation, retry state, deserialization and observer boundaries. Validated with the gRPC suite, formatting and static analysis.
A service deadline cancels the handler's pending wait. When its cleanup throws while cancellation unwinds, report that error and still return DeadlineExceeded for both unary and streaming calls. Preserve unrelated cancellation unchanged.

Inject the existing exception mapper into the inbound handler and update its provider and test construction. Document deadline interruption and finally-based cleanup, linking to the shared coroutine cancellation guidance.

Cover native and chained deadline cancellation as well as unrelated cancellation. Validated with the gRPC suite, formatting and static analysis.
A WorkerStopping listener, kill callback or exception reporter must not prevent Worker::kill from terminating the process. Use nested finally blocks to run the callback after listener failure and guarantee termination with the requested status even if reporting throws.

Extend the existing kill callback regression and cover listener and reporter failures through the worker's termination seam. Clarify shutdown behavior and that exceptions thrown after releasing an attempt do not count toward maxExceptions for the surrendered attempt.

Validated with the queue suite, including worker subprocess coverage, formatting and static analysis.
Integrate the framework metapackage, application skeletons and their installation and configuration checks, publishing workflow updates, Ollama finish-reason correction, and the recorded Sentry profiling regression.

The merge has no conflicts or overlapping changed files. Queue and signal defaults remain consistent with the worker timeout handling, and the split package installation resolves the new coroutine dependencies.

Validated formatting, framework and skeleton static analysis, generated configuration, the queue and Ollama suites, configuration generation tests, installed skeleton tests, and publishing script tests.
Rename the exception-chain detector to causedByCancellation(), use reportExceptions() for the reporting switch, and retain parentId() without its redundant pid() alias. Update all existing callers, tests and documentation together while preserving the native engine API and foreign-cancellation boundaries.

When a connection-release listener or AI broadcast producer throws during cancellation cleanup, cancel owned child work instead of draining it as an ordinary failure. Preserve the original failure through broadcast completion. Regression cases cover live children, native cancellation and throwing cleanup, plus disabling and restoring exception reporting.

Validated with the full composer fix workflow, including static analysis, parallel framework tests, Testbench and sample-package tests.
A callback can throw a cleanup error while unwinding coroutine cancellation. Checking only the outer exception allowed retry() to call the operation again and rescue() to run its reporter or fallback after cancellation.

Use the shared exception-chain detector before either helper continues. Extend existing regressions to preserve exception identity and verify that retry policies, delays, reporters and fallbacks are not called. The helper tests and full composer fix workflow pass.
Guzzle transport callbacks can fail with a cleanup error carrying cancellation. Converting a response-bearing transport exception first discarded that cause, allowing retries or ordinary response handling to continue.

Unwrap cancellation before response conversion and classify chained cancellation in synchronous and asynchronous retry and throw callbacks. Keep the existing Exception catch boundaries. Regressions cover both request modes, callback failures, and transport failures with success and error responses. HTTP tests and the full composer fix workflow pass.
Store callbacks can replace cancellation with a cleanup exception. Treat that chain as cancellation before attempting another store, recording failover, or continuing lock flushing.

Extend the existing first-success, every-store and lock-flush tests with chained failures while preserving ordinary failure suppression and events. Cache tests and the full composer fix workflow pass.
Recognize cancellation carried by cleanup exceptions in stack writes, per-key compensation and operations across all layers. Complete owned rollback while retaining the original cancellation; cancellation during cleanup still takes precedence over an ordinary failed write.

Extend the existing rollback and multi-layer regressions to cover chained failures without changing normal error precedence or continuing unnecessary work on an unavailable layer. Cache tests and the full composer fix workflow pass.
Credential cache reads, writes, removal and refresh-lock operations must not fall back to another store or a provider when cleanup carries cancellation. Classify the full exception chain and release an acquired refresh lock before propagating failure.

Preserve an existing cancellation when lock release also fails, while allowing release cancellation to supersede an ordinary provider failure. Regression cases verify provider suppression, lock availability, cache updates and both failure-precedence directions. Queue tests and the full composer fix workflow pass.
A canceled dispatch must not try another backend merely because cleanup replaced its outer exception. Detect cancellation before recording a failed connection or continuing the failover loop.

Extend the existing cancellation regressions to verify that no additional dispatch or failover event occurs and that earlier ordinary failure suppression remains intact. Queue tests and the full composer fix workflow pass.
Observer startup, engine work and completion callbacks can throw during cancellation cleanup. Recognize those chains before running further work or retaining an ordinary operation failure.

Extend existing regressions to verify that engine work and later observers do not run after cancellation, including when an earlier operation already failed. Scout tests and the full composer fix workflow pass.
A cleanup exception from the first-page handler could hide cancellation and allow the paginator to schedule all remaining pages. Detect the chained cancellation before retaining an ordinary callback failure.

Extend the existing real-coroutine regression with throwing cleanup and verify that only the first page is requested. Paginator tests and the full composer fix workflow pass.
Sub-agent and filesystem tools return useful error results for ordinary failures, but must propagate cancellation even when cleanup replaces its outer exception. Use the shared classifier before converting failures into tool output.

Extend existing synchronous, streaming and filesystem regressions to verify exception identity across cleanup chains. Include optional metadata paths that use rescue(). AI tests and the full composer fix workflow pass.
A cleanup error carrying cancellation must not be recorded as a completed tool failure or allow later approved tools to run. Classify cancellation before constructing and persisting an error result.

Extend the approval regression to cover native and chained cancellation, a second approved call, and suppression of follow-up model generation. Gateway tests and the full composer fix workflow pass.
Recognize cancellation hidden by cleanup before starting title generation or turning a title failure into a fallback. When title generation is canceled after a turn completes, persist the completed turn first and then rethrow the original failure.

Extend existing stopped-stream and title-cancellation tests to verify stored content, title fallback and exception identity with cleanup chains. Conversation tests and the full composer fix workflow pass.
Document when callback and listener boundaries need exception-chain classification, how shared failures retain their original cause without canceling other callers, and how direct engine coroutines contain cancellation after cleanup.

Require behavioral regression coverage for throwing cleanup while retaining valid native-only checks. Track investigation of focused PHPStan checks with explicit false-positive and ownership limits instead of attempting to infer all cancellation semantics automatically.

Propagate caller cancellation through job fetching and execution, including errors thrown during cancellation cleanup. Contain daemon-job cancellation at the owned coroutine boundary and report cleanup errors once with the worker context and reporting setting.

Apply timeout retry and failure policies when a non-kill handler catches cancellation without publishing an outcome. Wait for an in-progress timeout notification before choosing an outcome, so its release or deletion cannot race failure handling. Cancellation of that wait propagates without applying competing policies. Preserve already-deleted attempts as well as released and failed attempts.

Extend the existing worker regressions for cleanup chains, caught cancellation, notification release and deletion, both cancellation modes, exception accounting and marker ownership. Worker tests, queue unit and integration suites, formatting and static analysis pass.
Recognize cancellation carried by a cleanup exception when deciding whether to drain deferred callbacks. Preserve cancellation over an earlier ordinary command error when observers, mutex release or deferred work are canceled.

Keep the existing cleanup order and first-error behavior for ordinary failures. Extend the command and deferred-callback regressions with throwing cleanup and exact exception identity checks. Console unit and integration suites, formatting and static analysis pass.
Treat cancellation during scheduled output writes as an interrupted execution: release the owned mutex and process reference without starting completion callbacks. Ordinary output failures still run the established completion and cleanup sequence.

When later cleanup is canceled, preserve that cancellation instead of replacing it with an earlier ordinary error. Cover output cancellation and cleanup precedence through start, output and after-callback failures. Event regressions, console unit and integration suites, formatting and static analysis pass.
Rethrow cleanup-carried cancellation before converting a scheduled callback failure into an exit code. This prevents cancellation from starting onFailure callbacks.

Also preserve cancellation selected by the parent event during after-callback or mutex cleanup instead of replacing it with the original callback error. Ordinary callback-error precedence remains unchanged. Regression tests cover native cancellation and cleanup exception chains; console suites and static analysis pass.
Recognize cancellation through cleanup exception chains before reporting a failed scheduled task or draining its deferred work. Cancellation during deferred cleanup supersedes an earlier task error, while ordinary first-error behavior remains intact.

Extend scheduler regressions to verify mutex release, suppressed failure and completion work, and cancellation precedence during deferred callbacks. Console unit and integration suites, formatting and static analysis pass.
Document why background and deferred queue execution report cleanup errors through the configured exception callback while the job context is active. Native cancellation remains silent; moving cleanup reporting to the outer coroutine would bypass the configured callback and lose that context.

Extend the existing background cancellation test to verify one report of the original cleanup exception without job failure handling. Both drivers share this execution path. Queue tests and formatting pass.
Describe cancellation as starting catch and finally cleanup rather than guaranteeing that cleanup finishes before process termination. Explain that an attempt still unwinding after the bounded wait is terminated without concurrent job failure handling.

Clarify that emergency termination during a blocked event loop can occur before stopping listeners or a custom kill callback finish. Keep the detailed behavior in the canonical worker-timeout documentation and link to it from the shorter descriptions.
Contain failures from the configured exception reporter and fall back to the PHP error log, retaining both the original error and a distinct reporting failure. Keep cancellation from the reporter separate from a handler rethrowing an error that was already being handled.

Use the same reporting helper throughout Worker so a broken reporter cannot abandon other expired attempts, skip non-kill timeout policies, bypass lost-connection shutdown, or erase errors during payload handling and termination. Cancellation during non-kill reporting propagates without publishing completion or competing cleanup.

Extend the existing worker regressions for reporting failures, fallback diagnostics, original-error identity, cancellation during reporting, job-pop handling and lost-connection shutdown. Worker tests, the queue suite, formatting and source/type analysis pass.

Regarding the description-template check: this is maintainer-directed work. The external-contribution template requirements do not apply, as stated in AGENTS.md. The feature-specific description covers the problem, behavior, limitations, regression coverage and validation, so I am keeping that structure.
