Summary

Add named and prompted forks to Agent Host chats in the editor and Agents windows:

/fork keeps its immediate fork behavior.
/fork <title> creates a fork with the specified initial title.
/fork prompt <prompt> creates a fork and sends the prompt with attached context. VS Code's naming strategy can use the source title and new prompt.

The three completion entries have distinct localized descriptions. The title/prompt templates insert without submitting, show argument hints, and highlight the complete command prefix without adding command-token editing side effects.

Following review of the AHP companion PR, all fork variants use the existing fork capability. Optional titles and the existing initialMessage need no additional capability. Older hosts may ignore the new title field.

Initial-message failures appear as ordinary errors in the created chat, rather than a special creation RPC error. Admission-rejected initial messages survive reload; ordinary rejected turns retain their existing persistence behavior. Unknown transport/navigation outcomes show the underlying error and offer target inspection without automatically resending. Source-turn selection, model/agent inheritance, attachment handling, and source-owned draft cleanup remain intact.

The title-restoration repair drains the owning session's pending metadata and catalog writes before idle release. A controlled compatibility-storage regression reproduced a saved rename being replaced by the older cached title during reopening. The repair preserves title precedence and error propagation, scopes both active and passive catalog waits to the owning session, and leaves the existing restart E2E expectations unchanged.

How to test

In both window types, open an Agent Host conversation with at least one completed turn:

Accept /fork and verify it forks immediately.
Accept /fork <title>, enter a title, and submit. Verify the fork starts with that title.
Accept /fork prompt <prompt>. Verify /fork prompt is command-colored and the argument hint is separate. Enter a prompt, attach context, and submit; verify one new request in the fork with inherited history.
Type/delete arguments and edit the prefix; verify hints and normal editing remain correct.
Verify a rejected initial message appears as an errored turn in the new chat; after reload it is still visible, with no automatic resend.
Rename a titled peer chat, switch away and reopen it, then restart the host. Verify the renamed title is retained in both the chat and session catalog.

Before the repair, the controlled wrong-title reproduction failed twice; the permanent legacy release-barrier regression also failed before the fix.

Draft notes

Companion protocol PR: protocol: add optional initial chat titles agent-host-protocol#526. The title-loss repair adds no protocol fields or schema changes.
Cross-platform CI is still required, particularly the Windows/Electron scenario that originally failed. Local macOS passes are not a Windows-success claim.
Cross-platform Azure validation has not run; local Azure CLI authentication is unavailable.
A local replay attempt and lint invocation were blocked before execution by the previously observed shared Node/npm executable-permission issue. Both succeeded after restoring owner-execute permission.
Earlier live Agents Window validation covered hints, prefix coloring, and editing. This repair and main merge were validated with automated tests, not a new live UI run.

The unknown-outcome notification hid the underlying error, so definite pre-creation failures (for example "Wait for workspace setup to finish") were reported only as an unknown outcome. Include the cause in the message. Inspect Target waited for a chat that may never appear and then returned silently. Report that failure instead. Rejected turns were made durable for every chat so a rejected fork prompt would survive a reload. Ordinary rejections leave the message in the client's composer, so persisting them kept records nothing needs. Follow the resumable precedent on the error variant: the rejected variant now carries recoverable, producers default to recoverable, and only the create-chat initial message opts out. Fix fork CI regressions after upstream sync
