Description

What

Reduce recurrent First Value alert noise while preserving detection of sustained or alternating failures. A model's retry attempts remain one scheduled check. Operator summaries also retain retry-recovered degradation.

Why

The investigated nineteen-slot sequence produced two urgent alert/recovery pairs despite recovery at each next scheduled check. This policy produces one low-priority degradation summary for that sequence. Customer impact and the upstream cause remain unverified.

How

  • Confirm transient model incidents after two consecutive failed ten-minute slots or three failures in six slots. Require two adjacent clean first-attempt samples for recovery.
  • Persist slot deduplication, ordering, bounded history, summary limits, and episode writes in one locked Postgres transaction. A durable recovery boundary prevents closed-incident failures from reopening on one new failure; summary history remains intact.
  • Keep authentication, configuration, usage-ledger, unclassified and other cheap-path failures immediate. Missing/disabled/stale/future data cannot recover an incident, gaps break streaks, and fresh serious evidence wins a racing same-slot success.
  • Send low-priority degradation summaries at most once per six-hour policy window, with immutable counts on redelivery and no summary recovery notice. Model call budgets and the daily grounded-answer indicator are unchanged.
  • A separate test-only commit injects DNS answers for synthetic push hosts. Production URL/allowlist/address checks and dedicated security tests remain intact; assertions and committed timeouts are unchanged.

Apply additive migration 0105_first_value_health before activating this app through the normal release gate. Existing episodes need two clean samples; the new window invents no history. Retain the schema on app rollback and close pending summary episodes before reverting to a version unaware of that condition. No production migration, merge, or manual deployment was performed.

Sustained transient failures take about ten minutes longer to confirm, roughly twenty minutes worst case plus execution. The independent stale-monitor watchdog, daily digest and finer transport diagnostics are deferred; this does not fix upstream latency.

Related Issues

Follow-up to #649 (Reliability Indicators), implementing the owner's directly requested alert-noise investigation and fix. No issue is closed by this PR.

User-visible impact

Operators receive confirmed availability incidents and grouped low-priority degradation summaries instead of isolated urgent alert/recovery pairs. Customer-facing flows and model budgets are unchanged. Contributor reminder tests no longer depend on cloud DNS resolving reserved .test hosts.

Privacy-boundary impact

None to customer access, visibility, ownership, sensitivity, approval or externalization boundaries. New persisted state and notice context contain only synthetic outcomes, timestamps and bounded counts, without prompts, account data, provider payloads or credentials.

Checks run

  • pnpm typecheck, pnpm lint, pnpm format:check, pnpm db:check.
  • Both production builds with NODE_USE_ENV_PROXY=1 pnpm build --env-mode=loose.
  • All eight workspace V8 coverage suites passed on the initial implementation, with 8,707 tests including root contracts. The unchanged DOM test that exceeded five seconds under cloud load passed in an isolated web coverage retry with a 15-second local test timeout. Reports use the repository collector's merge/normalization logic.
  • Updated recovery-boundary regression: reproduced the failure before the fix; 12 domain tests and 24 storage/service tests passed afterward, including eight isolated Postgres contracts and a persisted boundary across reconnection. The 44 web alert tests passed.
  • The 53 reminder/DNS-security tests passed at the default timeout without RES_OPTIONS. No production/system DNS or Node upgrade is included.
  • pnpm fallow:coverage:check and FALLOW_AUDIT_BASE=origin/main pnpm fallow:ci passed. One existing informational CRAP note concerns the unchanged Phase Seven test body; no blocking regression or waiver.
  • Standard PR CI passed on latest head 5f202cd9cec6e03b2bbf9bcbc69511e39156d9e7: Quality (including real-browser contracts), Database (migration drift, full replay and eight Postgres contracts), Test/Fallow (8,710 workspace tests), and Chromium Instant matrix (28 contracts). Vercel, CLA and CodeRabbit status checks are green.

The standalone pnpm test and manual/visual verification boxes remain unchecked: actual test execution used workspace coverage and focused Vitest commands; no paid production probes or customer UI verification were performed. The cloud pnpm commands use pnpm_config_verify_deps_before_run=false to use existing dependencies.

Walkthrough

Adds scheduled-sample health tracking for First Value alerts. It classifies model outcomes, persists a bounded six-slot history, confirms availability episodes and recovery, and emits rate-limited degradation summaries. It also adds notification context, PostgreSQL coverage, and a deterministic DNS fixture for reminder tests.

Changes

First Value alerting

Layer / File(s) Summary
Classify outcomes and advance health
packages/domain/src/first-value-health.ts, packages/domain/src/first-value-health.test.ts, packages/domain/src/operator-alerts.ts, apps/web/src/lib/operator-alerts/first-value-outcome.ts, apps/web/src/lib/operator-alerts/first-value-outcome.test.ts
First Value outcomes distinguish transient model failures from serious failures and unknown results. Health state tracks six scheduled slots. Serious outcomes open availability immediately; failed outcomes open it after consecutive failures or three failures in the window. Recovery requires consecutive clean samples.
Persist health decisions and alert context
packages/db/migrations/0105_first_value_health.sql, packages/db/migrations/meta/_journal.json, packages/db/src/schema/app.ts, packages/db/src/schema/app/first-value-health.ts, packages/db/src/schema/app/operator-alerts.ts, packages/db/src/queries/operator-alerts/types.ts, packages/db/src/queries/operator-alerts/first-value.ts, packages/db/src/queries/operator-alerts/first-value-decision.ts, packages/db/src/queries/operator-alerts/drizzle-store.ts, packages/db/src/queries/operator-alerts/in-memory-store.ts, packages/db/src/queries/operator-alerts/first-value.postgres.test.ts, .github/workflows/reusable-verify.yml
The migration adds durable health state and alert context. The stores observe samples and apply decisions; PostgreSQL tests cover duplicate and older samples, persistence, rollback, and summary limits. The database workflow runs the opt-in PostgreSQL test.
Submit First Value samples through alert passes
packages/db/src/queries/operator-alerts/service.ts, packages/db/src/queries/operator-alerts/service.test.ts, apps/web/src/lib/operator-alerts/pass.ts, apps/web/src/lib/operator-alerts/pass.test.ts, apps/web/src/lib/operator-alerts/pass.integration.test.ts
Alert passes observe the classified First Value sample before processing readings. Tests cover repeated samples, alert and recovery delivery, policy-write failure, and grounded-answer episodes.
Render and deliver First Value notices
apps/web/src/lib/operator-alerts/channel.ts, apps/web/src/lib/operator-alerts/channel.test.ts, apps/web/src/lib/operator-alerts/notice-message.ts, apps/web/src/lib/operator-alerts/notice-message.test.ts
Notices can include the recorded sample time and counts. Degradation summaries are non-urgent and use low push priority. Other notifications retain the existing priority fallback.
Document alert policy and rollout
docs/adr/0265-first-value-model-alerts-confirm-scheduled-samples.md, docs/adr/README.md, docs/operations/synthetic-first-value-check.md
The ADR and runbook describe confirmation and recovery rules, summary rate limits, persistence behavior, migration sequencing, and rollout instructions.

Reminder test DNS fixture

Layer / File(s) Summary
Use deterministic DNS in reminder tests
packages/db/src/queries/reminders/test-fixtures.ts, packages/db/src/queries/reminders/reminders.test.ts, packages/db/src/queries/reminders/dispatch-revalidation.test.ts, packages/db/src/queries/mutation-outcomes.test.ts, packages/db/src/queries/phase-7-personal-os-e2e.test.ts, docs/local-development.md
The reminder test-service factory supplies a fixture DNS lookup when a caller does not provide an endpoint checker. Reminder tests use the factory, and local development documentation describes the DNS setup.
